**Systems & Health Navigation Specialist**

**1.0 EFT - 36.25 hours per week - 12 month term**

**$31.24 to 38.74 per hour**

**Summary of Position**
- A professional independent practitioner, fully accountable and responsible for their own practice, working within a multi-disciplinary team, works with individuals who access community health centre programs and services.
- Develops, implements, and evaluates services and program delivery in conjunction with the Program Manager.
- Provides a continuum of services and supports, with a strong emphasis on community health theory, health promotion, mental health and wellbeing, trauma, food security, basic needs security, complex care coordination, systems navigation, advocacy and outreach, individual counselling, and mental health group facilitation.
- The Systems & Health Navigation Specialist reports directly to the Program Manager and the general administrative direction of the Executive Director and is accountable to the communities we serve.

**KEY RESPONSIBILITIES**
- Mental Health/Wellbeing Programming and Supports
- Provide mental health/wellbeing services and supports within the scope of relevant professional standards including: group facilitation, individual assessment and counselling, referral and follow-up, advocacy and outreach, systems navigation, complex care coordination, community pantry coordination, and other relevant activities.
- Consult with other interdisciplinary providers and make appropriate referrals when the person’s situation requires supports beyond the scope of practice.
- Create, lead, and co-facilitate mental health groups from a trauma informed, culturally safe, and harm reduction foundations.
- Create, manage and supervise a therapeutic environment
- Provide coaching, counseling and direct treatment within scope.
- **Crisis-Intervention and De-Escalation**_
- Evaluate immediate safety risks, de-escalate and diffuse conflict in an assertive, timely fashion
- Actively participate in the Risk Response team of Youville and other committees as directed.
- Integrate trauma-informed/healing centered principles and strategies in preventing and diffusing situations
- Create and facilitate a calm, compassionate environment for those experiencing distress.
- Ethically navigate in a trauma informed client centred way client situations regarding Professional duty to report (Suicidal ideation, homicidal ideation, harm to a child etc).
- **Health Education & Management**_
- Provide the people we serve with education/resources related to health practices, illness prevention and navigation of the health care system to access health services.
- Complete intakes, assessments and coordinate service delivery plans as appropriate and within your scope
- Facilitate health education programs for individuals/families in the communities we serve.
- Collaborate with other employees as well as colleagues from other organizations to develop programming and supports to meet the needs of the communities we serve.
- **Program Administration**_
- Develop, implement and evaluate health programming and services within a relevant professional scope in the area of mental health and wellness.
- Provide support to students/volunteers as appropriate
- Maintain complete and accurate charts and medical records, and participate in chart reviews and case conferences.

**Qualifications**:

- Relevant combination of education and experience in the areas of social services, mental health or related field.
- Minimum 2 years’ of group facilitation and individual counselling in a community-based setting
- Minimum 2 years’ experience in systems navigation
- Knowledge of Adult Learning and Behavior Change Principles
- Experience working with youth, young adults and seniors an asset
- Fluency in French preferred
- Knowledge and understanding of diverse traditions and cultural practices with an emphasis on comfort and understanding of Indigenous practices.
- Valid Driver’s license and access to a reliable vehicle
- Criminal Record Check required
- Child Abuse Registry Check required
