**JOB PURPOSE**
Trailblazers Life Choices Inc. is a non-profit organization dedicated to providing services to those who are experiencing intellectual challenges, mental illnesses and physical disabilities. You as a part of Trailblazers life choices Inc. will be responsible for overseeing the day-to-day functioning of the individual(s) and staff within the department. You will plan, organize, direct, control and evaluate the delivery of care services within your department, ensure a safe work environment, drive productivity, and foster engagement among employees.

**DUTIES AND RESPONSIBILITIES**
**Program Development**:

- Participate in the development of program plans, facilitate plans for participants and provide input into participants’ needs-educational, emotional and environmental.
- Advocate on behalf of participants.
- Participate in meetings related to the needs of participants and the program.
- Identify and develop recreational activities to ensure maximum integration.
- Identify areas for program growth and development and make recommendations to the Executive Director.
- Participate in the development, implementation and monitoring of individualized plans.
- Encourage and assist individuals to participate in their daily activities.
- Provide supports in a manner which respects and honors individuals’ privacy and dignity.
- Facilitate, teach and assist clients in the development of an effective communication tool (verbal and non-verbal)

**Administration**:

- Maintain participant records and monitor staff.
- Review and train staff on completion of documentation related to program and client needs (medication, seizure logs, etc.).
- Adhere to licensing requirements.
- Ensure all logbooks are completed and relevant information recorded.
- Demonstrate leadership in the organization’s development.
- Facilitate and promote a safe environment for staff and clients.
- Conduct bi-weekly staff meetings
- Staff development/supervision
- Orient staff to program needs.
- Mentor and lead direct service workers
- Participate in staff meetings and planning sessions.
- Provide input into disciplinary process.
- Participate in employee evaluations.
- Facilitate problem solving and conflict resolution
- Promote, build and maintain a positive work environment.
- Bring issues regarding both participants and staff to the attention of the Executive Director.
- In conjunction with the Executive Director, delegate tasks to the staff
- Demonstrate leadership in the organization’s development.
- Communicate and liaise effectively with management and staff.
- Ensure completion and secure storage of records and documentation in accordance with agency policies and procedures and requirements of the Department of Family Services and Consumer Affairs.
- Ensure participants’ health and medical needs are attended to in a timely fashion, (i.e. medication and seizure reporting).
- In consultation with the Executive Director, ensure the safety of staff by providing awareness and training to identify signs of potential risks.

**Advocacy**:

- Promote the concept of inclusion, full community participation, individualized and pre-employment needs.

Please notice, these were main duties and responsibilities; however, additional tasks might be assigned. Plus, sometimes you might be asked to provide support to a participant.

**ACCOUNTABILITIES**:

- Complete programming and scheduling for clients and staff on a weekly basis
- Ensure documentation is completed and accurate on a daily basis
- Includes client journals, logbooks, medical reports, disciplinary reports, etc.
- Communicate to caregivers on a regular basis\
- Make sure all care plans are being delivered on a daily basis
- Coordinate and lead staff meetings on a bi-weekly basis
- Participate in DSW performance evaluations with the Executive Director
- Identify staff training needs and advise the Human Resources Manager
- Ensure staff is following all safety protocols, policies and procedures and treating clients with utmost care and respect.
- Identify areas of improvement and advise Programs Manager on a regular basis.

**QUALIFICATIONS**
**Mandatory**:

- Experience of work in a unionized environment.
- Positive attitude and belief that persons with disabilities have the right / responsibility to define and pursue their life choices, and pursue active participation and involvement in the community.
- Experience working with individuals with an intellectual, mental health or physical disability.
- Minimum of 2 years’ Management experience to provide direction and supervision to supervisors and direct service staff.
- Excellent interpersonal skills. Ability to motivate and encourage staff and participants/clients.
- Excellent organizational skills; excellent written and verbal communication
- Ability to work independently and as part of a team
- A valid Class 5 Driver License with clear driving abstract and reliable transportation
