**POSITION TITLE : Mental Health Recovery Worker / CASE Manager (Farsi Speaking) - 1 Year Contract**

**REPORTS TO : Program Manager, Case Management Programs**

**ABOUT MADISON**:
Madison Community Services (Madison) is a non-profit agency, which promotes the independence, health, recovery and community integration of persons with mental health challenges through the provision of mental health case management, supportive housing and a broad range of other programs and services.

**DUTIES, RESPONSIBILITIES, AND RELATED TASKS**
- Assist in the planning, implementation and evaluation of programs and services.
- Comply with the agency policies, standards, guidelines and procedures.
- Participate effectively in staff and other designated internal meetings.
- Carry out client screening and intake process.
- Complete assessments that identify clients’ needs, priorities and abilities through information obtained from the client, his/her formal and informal supports and the referral source.
- Provide direct service to clients related to the settlement process including assist in obtaining a permanent residence card; social insurance card, health card; help with registration in LINC/ESL programs; connection and referrals to community services specializing in serving newcomers.
- Provide feedback to the agency about client and community needs.
- Maintain confidentiality and comply with agency standards and all applicable legislation.
- Assist clients in identifying and advocating for their civil, legal, housing and other rights.
- Conduct mental health/settlement related workshops at the office or in partner agencies’ locations as required.
- Perform duties in a safe manner, in line with Madison’s Health and Safety policies and all applicable statutes.
- Participate in agency internal consultations and committees as requested.
- Performs other related duties as assigned by the Program Manager of the Case Management Programs.

**QUALIFICATIONS, KNOWLEDGE AND SKILLS**
- 3 years of combined experience in the mental health and settlement sectors.
- University degree or equivalent credentials that are relevant to mental health & addictions case management is preferred.
- Strong understanding of recovery values and practices.
- A demonstrated understanding of settlement issues.
- Cross cultural communication skills.
- Strong written and verbal English and Farsi communication skills is required.
- Dari and/or Pashto communication skills is an asset.
- Understanding of relevant provincial, and local legislation and government policies, procedures and practices pertaining to housing & services provided by the agency.
- Knowledge of community resources connected to the provision of mental health and related services.
- Demonstrated good interpersonal and networking skills.
- Demonstrated skills in assessment, planning, and problem-solving strategies.
- Use of appropriate skills and resources to intervene with aggressive and suicidal behaviour to maintain safety for all individuals.
- Knowledge of immigration process and the settlement services in Toronto.
- Strong computer skills.

**Job Types**: Full-time, Fixed term contract
Contract length: 6 months

Pay: $27.00-$28.00 per hour

Schedule:

- 8 hour shift
- Monday to Friday

Ability to commute/relocate:

- Toronto, ON M5B 0B2: reliably commute or plan to relocate before starting work (required)
