Lions Housing Centres Adult Day Club is recruiting for a permanent full-time **RECREATION COORDINATOR**

**HOURS OF WORK**: 60 Hours Bi-weekly $ 31,000.00 annually

**Responsibilities**:
To coordinate all daily routine activities related to the delivery of a Day Program to Members. To coordinate, develop and maintain all programs in consultation with the Manager. To be cognizant of members’ abilities and adapt programming accordingly. To provide a highstandard of program delivery and to maintain programs consistent with the organizational aims and objectives and in keeping with advances in current gerontological thinking.

**Job Requirements**:
Bachelor of Recreation Studies or a Recreation Facilitator for Older Adults Certificate.

Special courses in recreation, gerontology, program planning or group development and cognitive impairment is an asset.

Safe Food Handlers Certificate.

**Job Specifications**:
Experience Organizing and planning group activities for Cognitively impaired seniors in a related setting.

Knowledge Understanding of the aging process and of cognitive impairment.

Awareness of community resources.

Understanding of the group process.

Knowledge and experience in computer usage.

Skills Ability to plan, implement, evaluate, and adapt all programs.

Good communication skills, both written and oral.

Creativity and resourcefulness.

Organized Independent thinker and problem solver

Computer Microsoft Office and Great Plains

**KEY RESPONSIBILITY AREA**: **Administration of Program Activities**

Specific duties Co-ordinate daily attendance in conjunction with Transportation Company

Complete WRHA daily statistics.

Supervise and maintain attendance and accounts receivable records.

Prepare petty cash records;
Maintain daily forms, records and revisions as necessary.

Ensure each member has a complete and updated care plan, and progress notes. These are to be reviewed/updated quarterly.

Ensure Case-coordinators receive a postadmission report for their client within 8 weeks.

Ensure appropriate acquisition of necessary program and equipment supplies in collaboration with the Manager of Recreation.

Write progress notes on members as situations occur to be kept in patticipants individual files.

Preparation of Occurrence Reports and be in touch with the Manager of Recreation when such happenings occur.

Provide maintenance of food supplies and equipment and requisitions for same.

Monthly, prepare and forward CQI STATS and WRHA spreadsheet to Manager of Recreation.
- Update and monitor relevant medical information, i.e. medication reminders, diabetics, allergies.

**KEY RESPONSIBILITY AREA**:
Specific Duties Maintain transportation list and fax same to the transportation company weekly.

Be in contact with the accountant of the transportation company in regards to any discrepancies in the billing.

Keep daily journal of all transportation issues.

Report any information regarding transportation problems to Manager of Recreation.

**Intake Process**:
Specific Duties Review referrals.

Perform phone visits or meet with prospective members and/or family member at work location to determine if ADC programs can meet the individual’s needs. Provide information about the program, clarify objectives and expectations, and assess the individual for appropriate attendance.

Liaise with Case Coordinators re: referrals, discharges, issues, etc.

Maintain the wait list.

Input into the computer new member information, i.e. Great Plains, Excel, Kardex, update mailing labels and next of kin listing.

**Program Planning and Evaluation**:
Specific Duties Maintain the established recreation, social and specialized programs, promoting group interaction, participation, cognitive stimulation, individual accomplishment and enjoyment of purposeful activities.

Plan, organize and promote programs for weekly/monthly themes in consultation with the Recreation Facilitator and the Manager of Recreation. Define group and individual goals for all programs. Provide a monthly calendar for website and members.

Exhibit creativity and resourcefulnes in adapting existing program resources and design programs that address the capabilities of the seniors with Alzheimer’s type dementia.

Evaluate information obtained and assess members’ needs and capabilities and identify areas for improvement.

**KEY RESPONSIBILITY AREA**:
Specific Duties Ensure that the defined program objectives formulated for each member are implemented.

Conduct ongoing regular assessments of member’s progress and follow-up with family and case coordinator as needed.

Liaise with community groups and services to provide A variety of community opportunities for participants;
Maintain all program files and program plans.
