**Salary**: $15,254 - $19,685 annually, based on qualification and experience
Reports to: Housing Services Manager

Location: Primarily work out of the Alliston office (39 Victoria St. E), with occasional travel to Bradford West Gwillimbury; extensive travel throughout South Simcoe (New Tecumseth, Essa, Adjala-Tosorontio, and Barrie).

CONTACT serves as the Housing Resource Centre for the South Simcoe catchment area in the County of Simcoe. Our agency provides assistance to people at risk of experiencing homelessness, as well as those experiencing homelessness, along with assistance accessing energy support programs.

POSITION OVERVIEW:
CONTACT Community Services is seeking an experienced Housing First Case Manager to help develop and deliver intensive case management services to our clients in the South Simcoe catchment area. Working from a Housing First philosophy, the Housing First Case Manager will provide community-based support to individuals who have been experiencing chronic or episodic homelessness.

The goal of the program is to assist individuals in high/moderate acuity to provide hands-on, low barrier support and to empower them to maximize their potential and acquire and maintain appropriate housing. The position involves assistance with finding appropriate housing, apartment preparation and moving, as well as financial literacy development, community support network development and anchoring and advocacy to ensure their housing is a priority. In addition, the position requires knowledge of community and home-based mental health and addiction support for referral purposes.

In line with the Housing First model, the position will include direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case Management includes: (1) assessment of the participant to determine service needs; (2) development of a collaborative individualized care plan; (3) referral and related activities to help the participant obtain needed services; (4) monitoring and follow-up; and (5) on-going evaluation. *Case Management is on behalf of the participant, not to the participant, and is management of the case, not the participant.

Regular contact with other organizations engaged in outreach, such as the Canadian Mental Health Association of York Region, community partners and landlords is an important component of the program, and regular documentation an expectation.

ACCOUNTABILITIES:
1. Consistently exercises discretion and discernment to assess, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
2. Works with supervisory assistance to evaluate possible courses of conduct.
3. Conducts outreach to potential clients through community-based partners to promote referrals.
4. Maintains an active caseload providing interventions as needed, within the respective area of expertise and credentials; assures procurement of additional services as needed.
5. Acquires and provides to the housing team the detailed information regarding an assigned participant to establish the foundation for the individualized care plan.
6. Works with the participant on a day-to-day basis using professionalism, non-judgment and discretion to implement the individualized care plan, which may include brokered services from community partners.
7. Assists in development, implementation and revision of individual care plans; assures that services provided are specified in the care plan and monitors progress toward possible goals, if identified.
8. Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining care plan goals.
9. Engages in landlord communications and advocacy in regard to the client support available to help maintain housing.
10. Provides crisis management for participants; makes linkages for interventions as appropriate.
11. Attend participant community appointments for support, where applicable.
12. Maintains participant records in accord
