**Job Duties**
- Complete all assigned duties as a member of a case management team with a common interest in the client’s progress.
- Assume responsibility for counselling and completion of all required documentation for those clients assigned to his/her caseload.
- In conjunction with the client, ensure that an individual case plan is formulated for each resident and ensure that the case plan is monitored and evaluated on a regular basis.
- Ensure reports are uploaded to the CSC OMS data in accordance with our contract.
- Ensure that any violations of conditional release are immediately reported to the appropriate authority.
- Ensure that the security of the facility, its contents and all personnel are maintained.
- Assist in the recruitment, screening, training and supervision of volunteers.
- Prepare written and verbal reports as required.
- Carry out administrative duties of the agency as assigned.
- Answer inquiries from the public in accordance with agency policy and in a courteous manner.
- Familiarize him/herself with all agency policy and procedures.
- Ensures confidentiality of the client is maintained at all times.
- Ensures any maintenance or safety concerns are identified and appropriate action is taken to rectify them.
- Attend regular case conferences.
- Attend regular staff meetings.
- Attend training seminars from time to time.
- Completion of client Plan of Transition.
- Conduct institutional or residential visits when required.
- Liaison with Parole/Correctional Services personnel.
- Assumes front-line responsibilities as required.
- Familiarization with emergency evacuation and fire safety procedures.
- Completion of client intake and orientation.
- Assist, as assigned, in the orientation and training of new staff.
- Assist in the supervision and training of student co-op placements and volunteers.
- Participate in community education and public speaking as required.
- Other duties as required**.**

**Salary**: $46,000.00-$48,000.00 per year
