The Resident Care Coordinator (RCC)is accountable for creating a human habitat that supports quality of life for Elders and is designed to meet the needs of Elders, staff, and the community at Regina Lutheran Home (RLH).

Reporting to the Director of Care (DOC) the RCC works collaboratively with internal and external stakeholders in developing, implementing, and maintaining the highest standard of Elder care and services in accordance with RLH Mission and Vision.

As an experienced practitioner the RCC is responsible in a leadership role for the day-to-day activities of the neighborhoods such as providing specialized clinical expertise, co-coordinating of resident car and supporting and teaching to the care team (including residents and residents’ families) within a collaborative environment.

**RESPONSIBILITIES**
- Assess Elders and care requirements daily and monitor and assist Care Partners to perform required responsibilities to observe routines, reinforce best practices and identify concerns with care or service delivery.
- Assess Elders when care needs and health status changes utilizing tools such as the RRIT and MDS to offer recommendations to the DOC regarding presentation to the Program Access Committee and/or transfers based on care needs.
- Acts as home liaison to SWADD assessor coordinators regarding intake for Level II Elders.
- Be a professional presence at the home, modeling the approach that Care Partners are expected to follow when providing care, and actively overseeing and participating in the delivery of care, managing Elder’s needs as required (i.e., ensuring proper repositioning, adhere to TLR requirements, provision of ADL and care requirements, safety needs are met such as call bells in place, and other safety devices).
- Provide leadership by supervising, assessing, teaching, and assisting as required with all aspects of personal and nursing care delivery. Teaching includes providing “hands on” demonstrations of various procedures (i.e., TLR procedures, infection control procedures, nursing and direct care policies and procedures, maintaining safe environments and safe work practices).
- Work with professional nursing Care Partners (i.e., administration of medications and treatments to Elders) to support the delivery of care and service.

**REQUIRED EDUCATION AND EXPERIENCE**
- Graduate of approved School of Nursing (Degree preferred).
- Current registration with the CRNS or RPNAS.
- Formal training in supervisory practices or equivalent experience.
- Minimum 3 years experience in Special Care Homes/Long Term Care

- Demonstrated ability to respond quickly to emergency situations.
- An understanding of what creates a collaborative working relationship among all Care Partners and a demonstrated willingness to implement/support that type of working environment.
- Demonstrated ability for critical thinking, ability to problem solve, utilization of conflict resolution skills and decision-making abilities.
- Demonstrated sense of initiative and ability to work independently.
- Working knowledge of Eden Alternativephilosophy and Principles

**Job Types**: Full-time, Fixed term contract

**Salary**: $41.22-$51.41 per hour
