The Intensive Outreach Team is a newly funded community mental health program that provides direct service to
homeless individuals living with mental health, health, and addiction issues in the Mid-West & Downtown East
Toronto shelter system. These mental health issues may impact the individual’s ability to be housed, linked to
appropriate supports and services, attend to their recovery, or receive financial entitlements. The work is client
directed and done in partnership with the client. The team comprises a multidisciplinary team working in
partnership to provide a specialized service to clients. Our work is grounded in the lived experience of the clients
and the communities we serve and the principles of anti-racism, anti-oppression, equity, social determinants of
health, and excellence. We are agile, learning, and always willing to try innovative things. We provide high-quality
case management services and housing stabilization services to homeless individuals in the community with severe
& persistent mental health difficulties.

**What You Will Do**:

- The Mental Health Case Manager will work directly with clients to build client centered case plans that will

improve the quality of life for the clients in the program.
- They will report to the Manager of the Intensive Outreach Team.
- Provide outreach services to clients referred to the program. This will include locating, engaging, and

developing a helping relationship with clients and their referring agencies.
- Assist clients in the development and implementation of a care plan tailored to the client’s needs and issues

designed to improve their quality of life. Identify and refer to resources specific to those needs and issues.
- Act as a liaison and client advocate among those involved with the client’s care.
- including family, other caregivers, medical and other multi-disciplinary providers.
- Conduct assessments such as OCAN and provide timely and accurate.
- record keeping and reports as required.

**What You Bring to the Team**:

- A diploma/degree in social services/health with a minimum of five years of experience within the homeless

services sector, preferably in addiction & mental health.
- A clear understanding of PHIPA legislation and the organizational policies that relate to being a Health

Information Custodian
- Strong understanding of Anti-Black Racism, anti-oppressive/anti-racism perspective in the context of the social

determinants of health when working with clients and their support networks.
- Ability to integrate Anti-Black Racism, anti-oppression/antiracism, critical disability, 2SLGBTQ+ and trans
- positivity, trauma, recovery approach in support to mental health and strengths-based principles into practice of
the issues that impact homeless clients with mental health and addictions challenges, and an ability to work in a
respectful, non-judgmental, and consistent manner.
- Extensive experience conducting outreach to people who are homeless, or having worked with people who are

homeless in a community-based setting.
- Knowledge of community resources for homelessness, health, mental health and substance use
