**About FHC**: Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighbourhoods; Flemingdon Park, Fairview and Thorncliffe Park: FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Health Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement. We are primarily funded through the Ministry of Health and Long-Term Care/Ontario Health (formerly: Toronto Central Local Health Integration Network TC-LHIN).

Our approach to community health encompasses the social determinants of health which includes: education, employment, isolation, food security and social supports, and utilizes a community development model to promote health, prevent disease, and strengthen community capacity. **About the Position**:As part of Population Health and Wellness (PHW), the Community Health Worker
- _Emerging Needs_ rolewill be part of the Health Promotion (HP) team and will play a critical role in supporting emerging needs such as Vaccine Engagement gaps, supporting new community groups in our catchment areas (Romani, Afghan, Ukrainian, etc.) and will be responsible for the efficient and effective delivery of HP programs and services in FHCs catchment areas. The Community Health Worker -_Emerging Needs _will work from both sites and in the community to deliver an array of programs and services.
**Key components**:
- Bring a Community Engagement perspective to organizational planning within the PHW Team
- Support vaccine engagement initiatives across FHC’s catchment, including local retail spaces, high density neighbourhoods, vulnerable housing clients, appointment accompaniment, vaccine on-site clinic support, virtual community engagement
- Undertake a systematic outreach strategy to expand access to equity deserving groups, to foster linkage as well assess unmet needs
- Plan, develop, implement and evaluate HP strategies, programs and services addressing the social determinants of health
- Work collaboratively within a multidisciplinary environment in the delivery of activities and services across the organization
- Build and maintain partnerships with residents and other organizations and providers
**Employment terms**: Regular Full Time, 35 hours/week. Evening and weekend hours can be expected
**Salary**: $49,969 - $59,427 per year, along with extended health benefits and pension plan
**Reporting to**: Manager, Population Health and Wellness and/or designate
**Responsibilities include**:
**General**: 20% of role
- Produce evidence-based health education materials as required, and prepare and distribute relevant resources for Vaccine Engagement and Health Promotion
- Write reports, maintain files, and, as required, develop documentation/data bases necessary for evaluation and work plans and budgets
- Maintain knowledge of emerging populations and evolving needs of those within FHC’s catchment
- Other duties as assigned by Manager, Population Health and Wellness or designate
**Navigation and Case Management Support
- Intake **40% of role
- Intake of new clients, including registration and liaising with primary care providers
- Monitor internal referrals, and ensure client's exposure to all facets of the centre
- Refer callers and community members to appropriate community resources and services
- Participate in enhancing the centre's relationship with stakeholders and partnerships
- Work closely with primary care providers to outreach to community of clients
- Provide navigation support to clients, including information and referral to local services
- Provide brief crisis counselling to clients and community members
**Programming**: 20% of role
- Plan, implement, monitor and evaluate health promotion programs and strategies for emerging populations
- Use evaluation methods and reporting tools to gather and maintain statistical information on activities and programs
**Vaccine and Community Engagement**: 20% of role
- Meet community needs by connecting them directly to vaccination efforts, working closely with Community Ambassadors and closely with the local network of service providers.
- Work closely with system partners such as hospitals, Toronto Public Health and other community agencies to plan, support and evaluate community COVID19 outreach centres throughout FHC’s catchment areas
- Explore and expand community development approaches
- Establish advisory groups to involve members of the community in planning
- Act as a resource to the community and FHC staff by establishing links with external community groups, agencies and organizations
