**POSITION TITLE** Transitional Age Youth Coordinator (TAYC) Summer Employment

**LOCATION & LENGTH OF TIME**: 615 Industrial Drive, Fort Erie

**TITLE OF IMMEDIATE SUPERVISOR**: Family Support Case Manager

**PURPOSE OF POSITION**:
Under the general direction of the Manager, the Transitional Age Youth Coordinator will provide direct support to the individuals with intellectual challenges.

As the Coordinator, the responsibilities are (but not limited to)
- Assist with the recruitment of staff and participate in the interview process, follow up with completing references.
- TAY Coordinator will meet with each Transitional Aged Youth to complete and update their profile/goal sheets
- Complete/update resumes
- Setup binders for TAY coordinator/facilitator(s)
- Work with Family support case Manager to organize caseloads

Guide the program and the facilitator(s) to ensure they are meeting their targets and timelines
- Work with other agencies to form partnerships to foster current partnerships
- Meet with TAY participants, families, school representative to develop a plan of supports
- Meet with participants to obtain their goals/interests/likes to ensure we are investigating placements, that all are in line with their interests as best as possible.
- Setup and secure volunteer, job placements and job readiness training by accessing community programs and agencies
- Setup all support needs; setting up community transportation, accessing unemployment services, identify financial barriers etc., for participants
- Job search and approaches to community based businesses/ organizations to discuss possible work/volunteer opportunities available throughout the summer
- From May - June the TAY Coordinator is to investigate employment training opportunities/certificates and setup with agencies and TAY participants to attend
- Complete daily case notes
- Attend weekly staff meetings as well as take minutes and distribute weekly
- Attend meetings at schools and placements
- Arrange placement transportation with TAY participants/parents/caregivers
- End of month reports
- Ensure students have their school volunteer hours completed and school forms are signed by the event programmer. (These are to be handed to the school in September)
- Use computer to investigate community employment related opportunities
- Prepare task analysis of each TAY placement (both volunteer and employment)
- Have flexible work schedule around needs of participants schedule - ability to work weekends, evenings and holidays is required
- Coordinate employment/volunteer placements to be an ongoing part of the TAY’s transition plan within the schools when opportunities are possible.
- Provide hands on support at each placement as required until TAY is able to be independent
- Meet with Manager to review program
- Use personal vehicle to transport TAY when needed (transportation cost to be paid per kilometer - document mileage forms)
- Send out feedback forms to participants and families to hear back on the program with suggestions
- Keep required documentation for each TAY
- Obtain written recommendations from each volunteer/employment placement

**HOURS OF WORK**:
Up to 35 hours work week

**WAGE RATE**:
$22.50 per hour

**STATEMENT OF MAJOR RESPONSIBILITY**:

- Actively provide direct care by assisting, guiding, counseling, and supporting each individual in meeting their physical, intellectual, emotional and social needs through community participation.
- Facilitate daily living opportunities for each individual who are rich with community access, opportunity, dignity, stimulation and safety.
- Demonstrate a positive approach during all interactions to ensure appropriate role modeling.
- Adhere to all standards of regulations as dictated by any relevant legislation, including Occupational Health & Safety, the Services and Supports to Promote Social Inclusion of Persons with Developmental Disabilities Act 2008, Ministry of Community & Social Services and the policies, procedures, and guiding philosophies of the Association.
- Create personal profiles for each of the youth participants.
- Assist on developing Person Centered Plans to address the goals and interests of the individual.
- Promote and ensure the Agency’s Human Rights Statement and Commitment is followed when a person is unhappy with a restriction that is put in place.
- In conjunction with the support team, maintain all necessary records, files and charts as per established retention schedule, ensuring confidentially while providing adequate and appropriate communications with all people and services in meeting each person’s needs.
- Assist on seeking out training, education, potential employment & volunteer opportunities in the community and provide support as required.
