The **Direct Support Professional's **role is to support people to plan for and live the lives they want in collaboration with co-workers and the person’s support circle. They work as part of the team in problem solving and ensuring consistent and smooth operation of the home while following people's behaviour support plans. The Direct Support Professional will provide support in an environment of accountability, rights and respect that is consistent with Community Living Dufferin’s Mission, Vision, Values, the strategic plan, as well as relevant regulations and legislation.

**Key Responsibilities**:

- Actively assists people with daily planning and follows through with supports as directed by people's plans, while ensuring effective communication, documentation and collaboration as required.
- Encourages people to actively participate in their home and community and connects people to community resources and services.
- Supports people to be connected to family and friends as desired by the person.
- Supports people in assessing risk and developing a plan to help keep them safe, being aware of personal safety concerns in the process.
- Supports people through the choices they make and educates them on their rights and responsibilities that accompany those choices.
- Supports people to communicate in different ways so they are able to express themselves effectively (giving people a voice).
- Assists people with purchasing items they require (groceries, clothing, and other personal or household items).
- Supports people with money handling and budgeting skills.
- Supports people with their personal care routines which may include but are not limited to bathing, dressing, assistance in the washroom and with incontinence products, as well as other personal care procedures.
- Assists with cooking and cleaning routines in the home.
- Attends to medical needs, specialist appointments and health related supports (diet/nutrition, exercise) and ensures accurate records are maintained.
- Administers medication.
- Applies techniques of crisis prevention intervention and first aid/CPR as trained, when required.
