To help fulfill its mission, LOFT is hiring a** **Community Support Worker.**

**Job Summary**

The Community Support Worker creates a support network and links consumers living with mental illnesses to community services. This position provides a range of assistance and support including helping clients access and maintain housing. _*Work hours may include evenings and weekends._

LOFT’s Mental Health and Justice Initiative (MHJI) provides safe and dignified supportive housing for men and women, aged 16 to 64 who have a serious mental health issue, are homeless or at risk of becoming so, are currently involved in the criminal justice system and who can live safely and independently in the community with mínimal assistance.

**What You Will Do**
- Complete intake process for clients referred to the program and aid the development of plan of support and recovery
- Establish a holistic support network of social, family, spiritual, community, medical and psychiatric links and resources; assisting the individual in maintaining these linkages
- Support self-advocacy and advocating on behalf of the client in navigating the social services and health care systems and ensuring accessibility and availability of services
- Encourage individual participation and in house meetings, resident association activities or group activities to promote residents’ wellbeing
- Maintain and complete records consistent with case management best practices and standards

**What You Bring**
Must Haves
- Completion of a diploma/degree in Human Services or equivalent
- A minimum of 3 years’ experience working with the client population
- Knowledge of the issues faced by adults with mental health, justice issues and or addiction or homelessness
- Understanding of harm reduction, and psychosocial rehabilitation principles
- Excellent interpersonal, communication and counselling skills
- Knowledge of local community agencies and other professional services available for the client group is essential.
- Good intake assessment, crisis intervention, advocacy and problem solving skills.
- Computer literacy sufficient to produce and maintain files, records or statistical information
- Good analytical, time management, and organizational skills to contribute to program planning, co-ordinate and follow-up on issues or activities with clients
- Full license and access to a vehicle
