**The Community Outreach & Sales Manager, under the direction of the Area Director, is responsible for growing the client base and revenue within a defined geographic area, while ensuring compliance with internal and external standards of practice. The Community Outreach & Sales Manager develops and manages relationships with Business to Business (B2B) and direct to consumer (DTC) referral sources and must always present a professional image to the public.**

**DUTIES AND RESPONSIBILITIES**:

- Focus on sales opportunities, marketing, and community involvement. This position requires telephone and face-to-face sales calls with referral sources and direct interaction with clients.
- Travelling as needed when completing tasks related sales & marketing duties
- Establish and maintain relationships with referral sources, including hospital social workers and discharge planners, assisted living facilities, rehab facilities, physicians’ offices, fiduciaries, and B2B partners.
- Generate referrals for Bayshore by building vital business relationships, manages relationships with referral sources to promote and expand Bayshore Home Care services while leveraging all available technologies for increased effectiveness and efficiencies.
- Manage the process of converting a referral into an active home care client.
- Plan and host formal and informal caregiver support groups within a defined geography
- Promote Bayshore Home Health through participation in local healthcare or business committees as well as community events.
- Responsible for creating and establishing the marketing strategies and tactics for Home Care Services.
- Work collaboratively with team members to complete or arrange for the initial consult with clients and families requiring care and service
- Work collaboratively with team members to deliver great client and employee experiences
- Ensure care/service plans are available and updated, visit clients to ensure their satisfaction and link in other community resources and partners as required
- Document all Leads, Customers, appointments and events into our CRM system
- Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating bodies within the healthcare environment
- Participate in ongoing internal and/or external continuing education activities
- Adhere to Bayshore Policies and Procedures
- Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
- Participate in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
- Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
- Complete other tasks as requested

Job Qualification

**Education**:
Graduated from an educational program focusing on geriatric care, community health or social work is considered an asset.

Previous B2B and DTC sales training and or experience is required.

Previous training and or experience in marketing is an asset.

**Experience**:
At least 2 years of recent experience with responsibilities for client acquisition, care management and business growth, preferably in a community setting. Experience developing referrals and able to network within the assigned territory.

**Other Skills, Abilities and Requirements**:
Excellent planning, organization and communication & presentation skills are essential.

Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent manner; ability to work independently and as part of a team; above average report writing and communication skills; reliable vehicle and valid driver’s license; solid knowledge of the principles, practices, and methods of customer service, care management as well as program development, implementation and evaluation.

**Standards of **Performance**:
The Community Outreach & Sales Manager must demonstrate ongoing competency in completing all duties and responsibilities as detailed in this job description as well as agreed upon specific goals and objectives.

**Performance Review**:
Performance will be reviewed on an ongoing basis with a formal review at the end of the six-month probationary period and subsequently, on at least an annual basis. Input will be sought from the employee, Supervisor, Branch Office and in addition, will be based on ongoing client feedback and the extent to which performance meets expectations.
