**PRIMARY RESPONSIBILITIES**:

- Responds to requests to meet and discuss new family intakes; discusses background and special considerations and acquires an in-depth understanding of the children, family members and caregivers.
- Establishes relationships and builds trust with at-risk children, families and caregivers to assist with the unification or re-unification of the family.
- Meets with the children and caregivers to gain first-hand knowledge and understanding of the home environment; listens and responds to the individual experience of the children, family members and caregivers to determine what supports are required; creates a plan to maintain and support the requirements of the children and caregivers.
- Explains the role and responsibilities of the Outreach Coordinator to the family to ensure understanding and acceptance.
- Provides guidance and support to the children and family; advocates and/or connects individuals with additional resources (internal or external) to assist as necessary.
- Supports the education of the children by transporting them to / from school; attends parent/teacher discussions / interactions; advocates for parents/care-givers and children as necessary. Facilitates visitations from family members within TCOC’s facilities, family homes or other locations in the community; supervises the visits.
- Handles / de-escalates problems, issues and emotionally or behaviorally charged situations.
- Identifies risk to the safety and well-being of the child and reports concerns or issues to the Manager of Child Care Services - Family Support Unit and the Social Worker.
- Documents in-take, on-going outreach and visitations, risks and other pertinent information and forwards as appropriate.
- Assists caregivers with making required health or well-being appointments and may transport as necessary.
- Identifies and connects caregivers with tools and resources to assist or prevent escalation of issues (e.g. addiction counselling).
- Independently assesses risk with respect to visitations and contacts the Manager for on-site support as deemed appropriate.
- Connects with other community-based programs and people in support of the children and families well-being and participation.
- Identifies opportunities for inclusion/support of the Cultural Coordinator and brings these to the attention of the Manager of Child Care Services - Family Support Unit.
- Administers first-aid according to prescribed procedures; notifies emergency personnel as appropriate.
- Documents incidents and completes incident reports as necessary.
- Creates/maintains documentation in support of good program and plan management, evaluations and deliverables; provides reports and statistical information as required.
- Ensures the Manager of Child Care Services - Family Support Unit is kept informed of plans, activities, and issues.
- Participates in required training as directed by the organization; keeps current on relevant legislation, policy changes.
- Attends meetings, discussions and events as directed.
- Ensures internal policies, protocols and procedures are followed and incorporated into daily work practices.
- Maintains confidentiality and privacy of child/family information and models the organization’s values.
- Performs other related responsibilities as required

**QUALIFICATIONS**:
**Formal Education** - Completion of post-secondary education with a diploma in a related field of study from a recognized learning institution.

**Certifications/Training/Associations** - CPR-Level C/First Aid is required. Individuals must attend required training upon commencement (e.g. OH&S, crisis intervention, etc).

**Licenses** - Must possess a valid driver’s license and have access to a vehicle

**Experience** - A minimum of three years’ experience working work children who have special needs (e.g. trauma, physical or behavioral challenges)

**Knowledge** - Knowledge of child development and behavioral issues specific to children, underlying causes, and how to respond effectively Knowledge and appreciation of Indigenous history and culture Knowledge of internal policies, procedures and protocols Knowledge of addictions, other challenges and appreciates there are underlying issues and impact; knows where to source additional services to assist Knowledge of the Child Care Act, the Residential Services Manual and the Child Protection Services Manual.
