To help fulfill its mission, LOFT is hiring a** **Psychogeriatric Case Manager - Behaviour Transitional Support.**

**JOB SUMMARY**:
As a Psychogeriatric Case Manager, you will be part of a Behaviour Support Team that provides direct care, education and support to seniors who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions and who are ALC or at risk of becoming ALC in the acute care settings of the Ontario Health Central or Toronto Central area. You will assist in the creation and coordination of a behaviour support network, you will provide direct service to support the person served, family members and/or caregivers to mitigate escalating responsive behaviours. A key role of this position is to coordinate and support the transition of person served as they move to their next care destination.

**Note**:Occasional evening and weekend hours may be required. Frequent travel will be required using own vehicle.

**WHAT YOU WILL DO**:

- Assists persons served in the development and implementation of a Behavioural Support Plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours. Identify and refer to resources specific to those needs and _issues._
- Act as a liaison and advocate for the person served among those involved with the individual’s care including family, other caregivers, medical and other multi-disciplinary _providers._
- Complete PIECES/Functional Assessment and provide timely and accurate record keeping and reports as _required._
- Provide support, on-going training and consultation to colleagues, family members and/or other caregivers to assist in mitigating escalating responsive behaviours.
- Provide supportive counselling or referral and linkages to health care, programs or service _providers._
- Provide crisis management/intervention and assist in the development of crisis plans for persons _served._
- Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation _principles._
- Maintain strict confidentiality and follow all other required policies and _procedures._
- Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favorably on the organization and the Behavioural Supports Ontario _Project._
- Effectively participate as a member of the Behaviour Support Resource Team; attend staff/team meetings, care conferences for the person served, other multi-disciplinary meetings, committee meetings etc as _required._
- Attend and actively participate in all required _training._
- Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support _Services._
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption pursuant to the Ontario Human Rights Code in accordance with LOFT’s Communicable Diseases policy and program _guidelines._
- It is an expectation of the position that most of the work will be conducted in hospitals and compliance with their occupational health polices and protocols will be required (associated costs will be reimbursed)
- Complete other duties, from time to time, as assigned by the Coordinator and/or Program Director.

**WHAT YOU BRING**:
Must Haves
- Completion of a diploma/degree in Social Services/Human Services or _equivalent._
- Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological _conditions._
- An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with persons served on goals/_priorities._
- Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills
- Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive _training._
- Experience/familiarity with RAI documentation, and OCAN and health profession standards for documentation of information of the person _served._
