on and serves the spectrum of brain injury from mild to severe. *Role: *Reporting to the Program Manager, Services, the Information & Intake Coordinator is one of the first points of contact for those in the community impacted by brain injury who require information, education, or support with submitting a referral. They are responsible for providing support and resources, as well as screening for eligibility and assisting those ineligible to connect with the services that will be most helpful. This position will also provide case management to a small caseload of clients to assist them with identifying goals and obtaining the clinical and community support they need to reintegrate and regain independence. *Qualifications:

- * Certification: Related diploma or degree or combination of education and experience
- Experience: Case management or program coordination, project management, and/or public education
- Up-to-date and clear criminal record check *Major Responsibilities:* Intake & Eligibility
- Collect new referral information and any relevant medical documentation for all streams of service (Moderate-Severe Brain Injury, Concussion, Caregivers).
- Obtain and screen all new referrals for eligibility, investigate any potential concerns (i.e. history of aggressive behaviour) and recommendations to mediate potential concerns.
- Ensure that all client referrals are responded to and completed in a timely manner.
- Record referral and new client activity as per BCC Policies and Procedures in the applicable database.
- Liaise with community rehabilitation partners to determine future needs of clients prior to discharge from hospital.
- Provide resources and necessary support to individuals waiting for service.
- Continuously review the intake process and make recommendations for improvements in efficiency and client-centered practice.
- Meet weekly with the Lead Service Coordinator to review and assign new referrals. Education, Advocacy, & Outreach
- Coordinate the Public Education Program.
- Ensure education requests are received, prioritized and delivered to community partners.
- Facilitate or recruit a staff facilitator for educational sessions on brain injury for a variety of audiences and promote BCC programs and services to individuals who may require them, community members, and other stakeholders.
- Develop and distribute educational materials.
- Provide information about BCC services at community resource fairs and other venues.
- Maintain in-house communication materials such as brochures, website, and make recommendations for changes or updates.
- Have awareness of community and provincial resources available to individuals with acquired brain injury. Information Coordination
- Act as on-call Information Coordinator to provide information on brain injury and BCC services to clients, caregivers, and other professionals.
- Provide reception coverage when required.
- Use database to record community contacts, other relevant information.
- Provide assistance with advocacy and community referrals as required.
- Continually investigate community resources to better serve individuals with acquired brain injury and maintain/update the resource database. *Other Job Expectations:*
