The primary role of the 1FTE Social Prescribing Coordinator is to deliver initiatives that will promote social connections, enhance mental health and well being, create opportunities for community health and connection, while also building capacity within NorWest Community Health Centres and its service providers to address new and complex needs emerging from the pandemic. The Social Prescribing Coordinator will facilitate knowledge translation and process development during the project to embed social prescribing practices into the organization beyond the lifecycle of the project.

The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.

**QUALIFICATIONS**:

- An Honours Bachelor of Social Work
- Knowledge and experience working in collaboration with community health organizations to promote health and well being programs.
- Demonstrated values alignment with organizational vision, mission and values.
- Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
- Exceptional written, oral and presentation skills.
- Exceptional supervisory, organizational and time-management skills.
- Able to work independently and to effectively collaborate with community partners with excellent, networking, leadership, relationship building, interpersonal and listening skills to work effectively with a broad range of stakeholders and to facilitate the development of partnerships.
- Demonstrated commitment to the principles of equity, inclusivity, respect and cultural competence is required.
- Bilingualism (French/English) is an asset.
- A current driver’s license and access to a vehicle.
- Ability to travel to service delivery sites.
