**Position Summary**:

- Reporting to the Regional Director of the North & East GTA Stroke Network (NEGTASN), the Regional Stroke Rehabilitation and Community Re-engagement Coordinator (RCRC) supports the regional and provincial stroke rehabilitation, community, and long-term care vision of timely access to the appropriate intensity and duration of comprehensive, coordinated, expert rehabilitation and other stroke services across the continuum of care. Working with an interprofessional collaborative team approach, the RCRC will lead implementation of regional best practices for stroke rehabilitation, long term care and community re-engagement in collaboration with stakeholders, within the NEGTASN and Greater Toronto Area, across the continuum of stroke care. Communication and collaboration with many health care partners is integral to the position.
- Responsibilities for the RCRC include:

- Planning, coordinating, and implementing activities to expand linkages and partnerships with providers in rehab, long term care and community agencies within the network, raise awareness, and support the provision of integrated stroke care delivery based on best practices amongst providers in these sector.
- Enhance regional responsiveness to the needs of stroke survivors and their caregivers living in the community to support successful community re-engagement.
- Providing support to the development, delivery, follow-up and evaluation of educational initiatives for the rehabilitation, long term care and community sectors;
- Performing communication, coordination, measuring/ and monitoring responsibilities
- Support the development and implementation of best practice stroke care in the Rehabilitation, Long-Term Care and Community sectors within their region and across the Toronto Regional Stroke Networks. This may include engaging these organizations in the use of structures, processes, templates, and tools that support best practice.
- Develop and maintain an effective network of rehabilitation, long-term care facilities and community agencies in collaboration with the bordering regional partners of the Toronto Regional Stroke Networks
- Provide education on best practices in stroke rehabilitation, long-term care and community re-engagement at all phases of the continuum from acute care to the community.
- Active participation in the Ontario Rehabilitation Coordinators and LTC and Community Re-engagement Coordinators groups and work collaboratively with the MOHLTC, other coordinators across Ontario, the HSFO and Regional Partners in health promotion, stroke prevention, acute care, rehabilitation, long-term care and the community
- Building upon past work and projects with other regional partners, the RCRC will identify opportunities for system change, partnerships and consultation that will enhance access to stroke rehabilitation expertise and best practices.

**Qualifications**:

- Licensed member in good standing with a professional college and/or licensed member under the Registered Health Professions Act.
- Master’s degree (preferred) in any health related program such as Health Education, Rehabilitation, Nursing, Community Health, Epidemiology, Public or Health Administration or similar program
- Minimum of 2 years clinical experience in the health care system, with exposure to rehabilitation and community work.
- Demonstrated knowledge of neurological clinical content and the continuum of stroke care.
- Demonstrated coordination and networking skills.
- Strong consultative skills combined with clinical, organizational and rehabilitation community development knowledge;
- Previous experience as a change agent with an understanding of organizational cultures, including working with culturally diverse populations;
- Well-developed leadership, interpersonal, communication, problem-solving and conflict resolution skills, with the ability to work both collaboratively and independently;
- Experience in project management an asset.
- Strong oral and written communication skills.
- Experience in delivering educational presentations with demonstrated presentation skills.
- Computer literacy is essential, with the ability to use the internet, Microsoft Word, Excel, Power-point and Outlook.
