Qualicare Whitby is locally owned and managed by a nurse who has 24+ years of dementia, palliative and community care experience. We not only provide consistent, individual attention to ensure our clients and their families get the best care possible, but we are always available to support our care team.

Qualicare Whitby is looking for compassionate, caring and reliable individuals to join our family as a **Part-time Service Coordinator.** The Service Coordinator will work from home from 8:30am-4:30pm and will be responsible for working on-call, answering phone calls and managing scheduling concerns from 5pm on their working day until 8:30am the following business day. The Service Coordinator will also cover on-call responsibilities every other weekend and will be responsible for answering phone calls and managing any scheduling requirements that arise on the weekend while the Service Coordinator is on-call.

**The shift rotation will be from 8:30am-4:30pm 3 weekdays (business days) one week, then 2 weekdays (business days) and Saturday and Sunday on-call the next week, each working day including evening/overnight on-call responsibilities. The rotation will continue in this manner every 2 weeks.**

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
The role of the Service Coordinator involves jointly collaborating with and sharing responsibilities with the Director of Nursing, other Service Coordinators, Care Coordinator and an excellent team of community caregivers in the following responsibilities:
The **Service Coordinator** performs clerical functions, and provides client/caregiver support to ensure the appropriate service delivery to referred clients of Qualicare Family Home Care. This individual assists with the recruitment and onboarding of new staff, organizes and schedules referred clients, and anticipates staffing requirements to effectively manage client volumes. The Service Coordinator provides leadership and direction in all aspects of client care. The Service Coordinator functions and participates in the development, implementation, monitoring and evaluation of client services while upholding Qualicare brand standards. The activities are conducted in accordance with Qualicare’s policies and procedures.

**Responsibilities of the Service Coordinator will include**:

- Perform clerical duties to ensure efficient processing of referred clients
- Anticipate staffing requirements
- Effectively manages client schedules
- Timely response to clients, families, caregivers and referral partners
- Match clients to caregivers for customer satisfaction
- Inputs data into Qualicare's software platform
- Participates in the recruitment, hiring and onboarding of new employees

**Service Coordinator Requirements**

- Ability to work in a team environment
- Empathetic and compassionate with clients, families and caregivers

- Availability to occasionally work after-hours on-call

Though the position is a **work from home **opportunity, all training will be performed in the Qualicare Whitby office and successful applicants must be able to attend the in-office training in Whitby, Ontario.
