As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

**What will you do**:
Reporting to the Senior Manager - Home and Community Care, the Wound Resource Care Coordinator will promote and facilitate a best practice approach to wound and chronic disease management that is client-centered and evidence-based, in collaboration with the interdisciplinary team and our community partners.

KEY ACCOUNTABILITIES:
1. Assist in the development of policies and procedures for wound and skin management.
2. Community clinical consultation in wound and skin management. This responsibility includes direct client contact and interdisciplinary team consultation; direction and evaluation of individual care plans for wound and skin care.
3. Collaboration with community service providers, Primary Care Physicians and clinicians to ensure the continuum of care for patients and compliance with established wound care pathways.
4. Acts as a clinical resource for Care Coordinators and service providers; supports and reinforces established HCCSS Central East wound education programs/content with care coordination
5. Provides orientation to the HCCSS Central East wound care program for all new Care Coordinator staff.
6. Monitors adherence to wound care Pathways and Negative Pressure Wound Therapy units and protocol, utilizing reports
7. Will engage, partner and act as a liaison with Care Coordination, Physicians and patients to ensure that wound care best practice guidelines are adhered to.
8. Participates and initiates discussions and reviews of client wound care outcomes including team huddles, meetings and case reviews.
9. Reviews team documentation adherence, identification of challenges to team delivery of best practice care.
10. Demonstrates best practice in wound care; shares knowledge and clinical expertise with others through consultations, in-services, etc.
11. Participation with committees and regional/community committees, as appropriate, in consultation with the Home and Community Care Director.
12. Other duties as assigned.

**What must you have?**
- University degree preferred in Nursing, and /or equivalent
- Holds current appropriate unrestricted registration as a member of the College of Nurses of Ontario
- Registered Nurse in good standing with the College of Nurses
- Additional clinical qualifications in the area of wound management (e.g., Certified Enterostomal Nurse Therapist [CETN(C)]; The International Inter-professional Wound Care Course (IIWCC), or equivalent combination of education and experience.
- A minimum of two (2) years relevant clinical or community health experience.
- Demonstrated excellent interpersonal, communication, decision-making skills, and high flexibility is required.
- Ability to work independently and co-operatively in a busy multi-disciplinary situation.
- Knowledge of adult education principles.
- Ability to influence systemic change in a dynamic established healthcare environment.
- Experience in developing, documenting and implementing best practices.
- Actively engages in the identification of leading practice and evidence based data to support planning.
- Ability to deliver on organizational targets.
- Comprehensively use software including database, spreadsheet, word-processing, presentation, electronic mail and calendaring.
- Project management experience in the healthcare setting.
- Excellent understanding of Ontario’s healthcare system and healthcare administration.
- We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

**Hours of Work**

**Position Location and Travel**
- Valid Ontario driver’s license required.
- Travel within designated area to meet community groups, primary health care professionals and other health and/or social services.
