Brampton, Canada Queen Square Family Health Team Full time

_Queen Square Family Health Team (QSFHT) provides accessible, equitable, patient-centered primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors, and together, we serve 35,000 patients annually. We offer a wide range of programs and services including diabetes care, seniors’ wellness, palliative care support, mental health, maternal and children’s health and trans health. QSFHT is a dynamic organization meeting the community’s needs during the recent pandemic. We are a founding member of the Central West Ontario Health Team. **Responsibilities**
- Establish and implement a quality-focused plan that supports the FHTs in demonstrating effective practices, outcomes and value
- Lead and manage healthcare projects from initiation to completion, ensuring they are delivered on time, within scope, and budget.
- Collaborate with stakeholders, including clinicians, administrators, and external partners, to define project objectives and deliverables.
- Develop detailed project plans, including timelines, resource allocation, risk management, and communication strategies.
- Monitor project progress and performance, ensuring adherence to quality standards and regulatory requirements.
- Facilitate cross-functional team meetings, provide regular updates to stakeholders, and resolve project issues promptly.
- Utilize project management tools and methodologies to streamline workflows and optimize efficiency.
- Stay informed on healthcare regulations, trends, and best practices to guide project decisions.

**Qualifications**
- Bachelor’s degree in Healthcare Management, Business Administration, Project Management, or a related field.
- Proven experience in project management within the healthcare sector (e.g., hospitals, clinics, insurance, or biotech industries).
- Strong knowledge of healthcare regulations (e.g., HIPAA, CMS guidelines).
- Proficiency in project management software (e.g., MS Project, Jira, or Asana).
- Excellent communication, leadership, and organizational skills.
- Knowledge of one or more EMR/EHR systems currently in use in Ontario is considered an asset, preference given for knowledge of Accuro.
- Proven ability to develop and maintain strong relationships with partners
- Highly effective communication, presentation, and interpersonal skills
- Strong report writing skills
- Ability to handle a variety of concurrent assignments and to work independently
- Detail-oriented with quantitative analysis and problem-solving skills.
- See value and able to work effectively as a collaborative team member
- Demonstrated leadership skills, critical thinking and is proactive in their work
- Exceptional organizational and time management skills
- Requires flexibility in hours as the position entails some evenings and weekend work.
- Experience working with confidential and time sensitive documents and information

Queen Square Family Health Team offers a competitive salary and comprehensive benefits package, including medical and dental benefits and is a participating employer of the Healthcare of Ontario Pension Plan (HOOPP). We also offer an attractive and rewarding work environment.

Pay: $72,000.00-$80,000.00 per year
