Reporting to the Manager, Specialized Geriatric Services, the Registered Practical Nurse (RPN) at Manor House Adult Day Program is responsible for monitoring clients' health status, administering minor medical care, and consulting with care partners and other health professionals. In conjunction with the inter-professional team the RPN is also responsible for assessing clients, developing a plan of care, implementing interventions, providing comprehensive programs and activities, monitoring client responses, and facilitating the team to ensure client goals and needs are met.

**Qualifications**:
**Education/Experience**:

- RPN diploma from a recognized educational institution
- Current unrestricted registration with the College of Nurses of Ontario (CNO)
- Current Basic Cardiac Life Support (BCLS) and Standard First Aid certification
- Previous nursing experience working with older adults with dementia, mental health, and other neurological conditions, and those with acute behavioural changes, preferably in a community setting
- Demonstrated knowledge of dementia, delirium, mental health issues in the delivery of care and its effect on the person and family members/care partners
- Experience in health and behavioural assessment, symptom management, medication assessment and monitoring, health teaching, and supportive counseling

**Skills/Abilities**:

- Ability to work effectively in an inter-professional team environment
- Demonstrated leadership skills, including the ability to motivate and share skills and knowledge with others
- Ability to communicate clearly and concisely, both verbally and in writing
- Ability to use independent judgment and maintain confidentiality of records, actions, and information
- Demonstrated knowledge of local community resources

**Assets**:

- Certification relevant to Gerontology, and Dementia, including training in P.I.E.C.E.S.and Gentle Persuasive Approaches is an asset
- Membership in a professional association (ie RPNAO) is an asset
