**The Alzheimer Society of Muskoka is currently seeking applicants for**:

- **Support Counsellor - 24 Month Contract**_

The Alzheimer Society of Muskoka has received a Resilient Communities Fund Grant from the Ontario Trillium Foundation to support community-based organizations that deliver programs and services in Ontario to recover and build capacity, resilience, and sustainability. The grant will cover a 24-month period commencing in June 2023.

The Alzheimer Society of Muskoka (ASM), serving the communities of Muskoka and Parry Sound, is dedicated to improving the quality of life for individuals living with Alzheimer’s disease and other dementias and their caregivers. We are committed to providing advocacy, a caring and understanding support system, individual, family and group counselling, public education.

The Alzheimer Society of Muskoka offers a wide range of programs and services addressing the progress of Alzheimer disease with support for caregivers and families including supportive counselling, public education, weekly drop in programs, in home recreation, a resource lending library, music and Minds in Motion.

The Support Counsellor provides information, supportive counselling and referral services to persons with Alzheimer’s disease or other dementia and their family members & or informal care providers.

Supportive counselling provided by the Support Counsellor is a safe and confidential space for individuals and/or families to ask questions about dementia and/or caring for someone with a dementia, opportunities to discuss coping strategies and to obtain information.

**KEY RESPONSIBILITIES**:

- Good knowledge of ASM Mission, Vision and Values
- Compliance with Privacy Policy; obtains and records consents
- Facilitate and support ASM Support Groups
- Collaborate with Recreation Support Facilitator to provide Support Group respite program
- Assist with coordination and facilitate Drop-In program as needed
- Attend satellite office in Parry Sound to provide support services as available
- Identify support service needs for persons with dementia and their care partners
- Assist with the monitoring and evaluation of the Supportive Counselling program to ensure it is meeting the needs of the target group
- Collaborate with FLCN for receiving new referrals and completing ASM ‘Intake’ process if required
- Provide input into the development and ensure implementation and evaluation of the First Link ® program
- Provide information and supportive counselling to care partners and persons living with dementia through office visits, home visits (when necessary), telephone consultations, virtual meetings (Zoom/OTN) and follow-up following Support Services policies and procedures
- Liaise with and link clients to AS education and support services, and to appropriate community agencies and resources based on needs and requests in a timely manner
- Work with the Public Education Coordinator in the planning and promotion of the First Link® Learning Series
- Support clients in navigating the healthcare system to access appropriate learning opportunities, support services, care, and resources as identified in their individualized plan of service
- Provide input into the development of ASM annual work plan and strategic direction
- Develop and implement an annual Supportive Counselling work plan.
- Maintain and manage client files/caseload and statistical data in accordance with ASM funding and policy requirements
- Maintain current knowledge of dementia and relevant community resources
- Be prepared to upgrade skills as required
- Supervise ASM volunteers who assist with the Teleconnect volunteer program
- Always represent the Alzheimer Society of Muskoka in a professional manner and act in the best interest of the organization
- As an employee of a non-profit organization, you are expected to participate in fund development activities as required
- Perform other duties consistent with the job classification, as required or assigned

**QUALIFICATIONS**:

- Post secondary degree in social work (preferred), social services, health services or related discipline or equivalent combination of experience and education
- Minimum 3 years experience working with persons with dementia and/or their family members/caregivers
- Experience with individual, group, and family counselling with strong knowledge of client-centred philosophy
- Strong group facilitation skills and experience
- Comprehensive knowledge of and understanding of Alzheimer’s Disease and other related dementias
- Knowledge of available community services/supports and clinical, social, and residential care options
- Understanding of roles and linkages across primary care, community care, specialized geriatric services and acute care
- Exceptional interpersonal and communication skills (verbal and written), including shared decision making and facilitation
