**QUALIFICATIONS**
- Master’s degree in social work;
- In good standing with their professional association and have maintained their practicing license;
- A background in/knowledge of health services research, health systems administration and/or the medical field a strong asset;
- Experience working with youth in a social worker capacity;
- Strong active listening skills and ability to demonstrate empathy while remaining professional and respectful;
- Excellent communication, counselling and teaching skills, including the ability to act with tact, diplomacy, professionalism, and handle sensitive information with discretion;
- Experience establishing and maintaining relationships with diverse groups of stakeholders (physicians, scientists, donors, industry representatives, patients, etc.);
- High emotional intelligence to adapt and support the diverse personalities participating in the trial;
- Strong analytical abilities with knowledge and skill in critical thinking and decision-making;
- Excellent organizational skills, including the ability to organize workload, set priorities and respond effectively to competing demands;
- Strong aptitude in computer skills, including but not limited Microsoft Office Suite, REDCap, video conferencing software (Zoom, FaceTime, Teams, etc.), experience with messaging software (WhatsApp, iMessage, Signal, etc.)
- Self-motivated, ability to work under pressure and meet deadlines.
- Prior experience with individuals with chronic illnesses and/or with biopsychosocial models of care as social worker an asset;
- Open to flexible hours with evenings.

**RESPONSIBILITIES**
- Primarily responsible for supporting participants recruited to the PACE Transition Trial assigned programs - provides the administration, management and execution of all the transition intervention program and its multi-modal components, as per the pre-specified plan described in the research protocol and approved by site research ethics boards;
- Serves as a Navigator for patients to liaise with their pediatric IBD care team (physicians, nurses, etc.) and adult IBD care team/other healthcare teams;
- Proactively and collaboratively supports young adult patients and their families, as per the pre-specified plan;
- Acts as a primary source of contact about transition from pediatric to adult care for patients;
- Responsible for assessments of patients’ transition readiness including but not limited to, physical, social and psychological needs, and tailors educational and skill-building resources for patients according to pre-specified plan;
- Responsible for identifying urgent medical, mental health, and social situations and liaising with the health care team for prompt intervention;
- Ensures continuity of care as patients transition from pediatric to adult care, in collaboration with IBD health teams at pediatric and adult clinical sites;
- Communicates regularly with patients, either by telephone, texting or other digital platforms, to ensure that patients can access them with questions or concerns regarding their care;
- Participates as an integral member of the PACE transition intervention program team;
- Reviews data for program evaluation and makes recommendations for program improvement; and
- Integrates research into practice and ensures that best care practice standards are maintained;
