**Position**

The **Manager, Chronic Disease Management** is a member of the management team for the Edmonton North Primary Care Network (PCN), and has overall responsibility for the development, delivery, and oversight of the related roles, services and programs our organization offers to clients pertaining to chronic disease and efforts to improve client needs from primary care. This role works in collaboration with the other Managers and Rundle Health Centre Managers to support clinical services and programs and helps ensure that those the PCN provides are effective and efficient in supporting the needs of its clients.

**Key Accountabilities**:

- Ensure the purpose and goals of programs and services are defined and aligned with the organizations vision, mission, and values.
- Define the scope of clinical roles based on disciplinary core competencies to optimize multidisciplinary teamwork.
- Ensure programs and services align with the PCN’s strategic and business planning.
- Conduct environmental scans, gap/ needs assessments to understand changing and emerging needs.
- Collaborate with stakeholders to identify opportunities for improvements/ optimization and determine future direction.
- Plan, develop, pilot, and implement improvement initiatives and set goals, objectives and milestones related to same
- Engage staff in clinical program and service planning activities, among others

**Education**

Master’s Degree preferred

Bachelors Degree in Health Sciences or a health-related field required

**Experience**

Minimum of five (5) years of management experience leading clinical teams and overseeing related programs

Proven experience in development, implementation, and monitoring of clinical services, using evidence based measures

Demonstrated ability to lead organizational and program change initiatives

Experience working with multiple and diverse stakeholders

Primary Care and/or community health experience is a significant asset
