We are currently looking for a **HSW/PSW Educator** **that is a R**egistered Nurse (RN) or Registered Practical Nurse (RPN). **In this role, you will be responsible for planning, leading, implementing and evaluating a comprehensive education program for Home Support Workers and Personal Support Workers. You will work in collaboration with the leadership team, Client Service Managers, Home Support Supervisors and others to promote excellence in the delivery of high quality, safe, patient-centered care. You have a reputation for setting and achieving high standards for yourself and mentoring peers to achieve success. An energetic person who takes pride in building strong and effective interpersonal relationships, you bring to this role your knowledge of best-practices in caring for home health patients and you are motivated by and focused on achieving excellence.

**Job Description**:

- Develop, plan, deliver, and evaluates competency based education programs for Home Support Workers and Personal Support Workers
- Conducts needs assessments, provides recommendations and plans education programs to support strategic objectives and evidence based practice.
- Collaborates with Managers, Supervisors and teams to identify learning needs and assists in developing strategies to meet those needs
- Acts as a resource to support staff to identify and meet their learning needs
- Create learning environments that support professional growth and accountability in a variety of settings (clinical, classroom, simulation, etc.)
- Monitor and evaluate integration of learning into field practice
- Provide coaching and mentoring to staff; have insight and courage to address quality and risk issues in a timely and constructive manner
- Stays current with knowledge about trends in health care, professional practice, legislation and education

**Qualifications**:

- Post-secondary degree or diploma with current registration with the College of Nurses of Ontario (RPN or RN)
- 2+ years’ experience in a related educator role preferred
- Preferred minimum 3+ years home care experience
- Must have a vehicle and be willing to travel between locations within GTA
- Demonstrated knowledge of the principles of adult education
- Demonstrated ability to design curriculum
- Excellent facilitator with demonstrated presentation skills
- Proven commitment to evidence-based practice and reflective practice
- Demonstrated leadership skills and proven effectiveness as coach and mentor
- Knowledge and comfort with mobile technology and documentation
