The **Short-Term Mental Health Case Management Outreach Team** is a newly funded community mental health program
that provides direct service to homeless individuals living with mental health, health, and addiction issues in the Mid
- West & Downtown East Toronto shelter system. These mental health issues may impact the individual’s ability to be
housed, linked to appropriate supports and services, attend to their recovery, or receive financial entitlements. The
work is client directed and done in partnership with the client. The team comprises a multidisciplinary team working in
partnership to provide a specialized service to clients. Our work is grounded in the lived experience of the clients and the
communities we serve and the principles of anti-racism, anti-oppression, equity, social determinants of health, and
excellence. We are agile, learning, and always willing to try innovative things. We provide high-quality case management
services and housing stabilization services to homeless individuals in the community with severe & persistent mental
health difficulties.

**What You Will Do**:

- The Housing Worker will help build housing first case plans for clients that are homeless and experiencing

complex mental health issues.
- Provide outreach services to clients referred to Short Term Mental Health Case Management Team. This will

include locating, engaging, and developing a helping relationship with clients and their referring agencies.
- Assist clients in the development and implementation of a care plan tailored to the client’s needs and issues

designed to improve their quality of life. Identify and refer to resources specific to those needs and issues.
- Act as a liaison and client advocate among those involved with the client’s care, including family, other

caregivers, medical and other multi-disciplinary providers.
- Conduct assessments such as OCAN.
- Record keeping and reports as required.
- Engage with the tenant/clients to explain the assessment process and the role of the Housing Worker.
- Assess the tenant/client’s community support needs and wishes (i.e. daily living skills, housing, finances,

social networks vocational and educational goals and environmental resources).
- Conduct comprehensive psychosocial assessments with clients who may be eligible for housing.
- Based on tenants/client’s needs and goals, develop a recovery oriented plan with the tenant/client and

provide support services which include but are not limited to: mental health support, independent living
skills, budgeting, social skills teaching, social recreational activities, advocacy and family education, crisis
management, and discharge/termination of service plans.
- Work collaboratively with the tenants/client’s treatment support network and other relevant service

providers to create Mental Health Support, Crisis Plan and follow-up.
- Provide crisis intervention when needed.
- Work as a member of a team with other Housing Support Workers, Mental Health Court Workers, Case
- Managers and other community workers to ensure the program operates efficiently and effectively.- Refer and link clients to appropriate mental health and other community services as needed.
- Maintain program and agency documentation as per policies.
- Maintain confidentiality as per “Personal Health Information Protection Act.”
- Work with the Program Manager to identify and meet professional development needs.
- Participate in agency and other meetings and initiatives as appropriate.
- Develop and maintain collaborative relationships with key supports and resources in the community.
- Develop and maintain open and effective working relationships with lead partner agencies.
