**Position Overview**:
The Edmonton Southside Primary Care Network has an exciting opportunity for a full-time, Exercise Specialist (ES). The ES will provide comprehensive exercise services and care to patients within the Primary Care Network. The ES will assess, plan, implement and evaluate physical activity and fitness-based care plans to patients of all ages. The ES will be an integrated team member working to their full scope of practice within the primary care setting.

The Exercise Specialist will report to a Primary Care Manager

**Key Roles and Responsibilities**:

- To work collaboratively in a patient centred, integrated team based primary care practice setting.
- To participate in the care of individual patients, practice populations and associated communities.
- To evaluate patients’ fitness and readiness for change, prescribe conditioning exercise, and provide exercise supervision, health education and follow-up assessments as needed.
- To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team.
- To act as a physical activity resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.
- To ensure accurate, objective and timely clinical documentation in the appropriate medical record.

**Experience, Skills & Qualifications**:

- Baccalaureate degree in Physical Education, Kinesiology, or equivalent
- Canadian Society of Exercise Physiology Certified Exercise Physiologist (CSEP-CEP) or American College of Sport Medicine Clinical Exercise Specialist (ACSM CES) Certification
- Current Basic Life Support (BLS) for Healthcare provider
- Vehicle/license - when appropriate
- Previous experience:

- Primary care setting
- Electronic Medical Record
- Group facilitation/instruction
- Health behavior change methods (eg Motivational Interviewing)
- Understand the Alberta privacy laws and act according to those laws.
- Should behave responsibly, be accountable and have excellent written and verbal skills, problem solving/critical thinking skills.
- Experience facilitating group classes.
- To support the vision and mission of the Primary Care Network.
