Reporting to the Manager, the Intake Clerk works as part of the Oncology Patient Navigation Program, which aims to improve the quality and accessibility of the diagnostic phase for all individuals undergoing a potential cancer diagnosis. The Intake Clerk is responsible for liaison between referring physicians, medical specialists, and health care organizations of Southwestern Ontario. The Intake Clerk receives and processes referrals, compiles health information in accordance with disease site guidelines and schedules appointments. The Intake Clerk is expected to ensure that all relevant information related to the cancer diagnosis is obtained and available in the patient’s chart in order for the care team to determine the optimal scheduling needs for the patient.

Regular Part-Time

Rate of Pay: $26.44 per hour - $28.63 per hour

Hours of Work: 22.5 hours per week

**Qualifications**:

- Successful completion of a Two-Year Diploma in Medical Office Administration
- Minimum of 2 years Clinical Experience in Oncology preferred
- Previous experience with patient registration and scheduling preferred
- Proficiency in computer and keyboarding skills, i.e., Microsoft Office (Word, Excel, Outlook), Electronic Patient Record, Workload Measurement
- Demonstrated proficient communication skills (both written and verbal)
- Effective organizational skills involving time management, prioritization and the ability to problem solve while ensuring attention to details
- Demonstrated flexibility, adaptability and ability to manage change in a dynamic environment
- Demonstrated ability to work well and independently with minimum supervision in a team environment
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC’s vision, purpose and values
- Demonstrated ability to attend work on a regular basis

Before beginning employment at LHSC, all new hires will be required to:

- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;
- Provide documentation of the Tuberculosis skin testing (two step);
