Reporting to the Director, Community Health & Chronic Disease (CH & CD), the Program Manager - Community Health is responsible for managing the community health/ community health promotion team and programming across SRCHC. This includes managing the Harmony Community Food Centre and food access programs across the organization, 4 Seniors Active Living Centre Programs, a Toronto Ride Seniors Transportation Program and a Community Health Promotion program. within SRCHC. This position provides operational oversight for services at 955 Queen Street, 2 Gower and Crescent Town (4 The Market Place).

This position ensures high-quality services and excellent customer service in all program areas in particular the Seniors Active Living Centres (SALCs), Seniors Transportation services, Health Promotion programs, Food Service to other agency programs and Community Food Centre that meet the needs of service users. In this role, the Program Manager is further responsible for the supervision and support of a diverse team of staff, volunteers and students, including hiring, performance management, disciplinary action and decisions regarding termination of employment.

This position is expected to work onsite during the COVID-19 pandemic as required.

**Responsibilities**:

- Responsible for planning, coordinating and supervising the development, implementation, evaluation performance management and funder reporting of all designated programs
- Develop comprehensive understanding of direct service provision and provide support as needed
- Ensure principles of Health Promotion are incorporated in program activities along with the values of SRCHC
- Assess program results on an on-going basis and take appropriate action to ensure the required quality improvements are made
- Ensure programming and services reflect best practices and make best use of limited resources
- Monitor collection and reporting of statistics and other program data
- Collaborate with the Director, Quality, Accountability and Evaluation to evaluate program current practices and capacities to meet goals of improving services
- Write & ensure all reports related to funders are completed in a timely manner and submitted to Director on schedule for review and submission
- Accountable for ensuring program targets and goals are met
- Coordinate grant proposal writing with Management Team in consultation with Director, CH&CD
- Oversee strategy and outreach for community, financial or in-kind donations
- Ensure all relevant administrative systems are maintained accurately
- Develop, review and monitor program budgets in consultation with Director, CH &CD and VP, Strategy & Systems
- Forecast when vehicles need replacement
- Identify program policy development needs and oversee development, updating and implementation of policies
- Identify emerging needs and priorities for optimizing client primary health care, case coordination, community programs and outreach services within the operating plan for SRCHC’s priority populations
- Facilitate and develop team work plans (in consultation with management) that provide input into planning and reporting infrastructure
- Direct and manage the work of interdisciplinary teams (including students and volunteers)
- With support from the Director, Community Health & Chronic Disease, conduct all required recruitment activities to ensure the programs are appropriately staffed
- Provide supervision and ongoing guidance, feedback, recognition and coaching of the program staff
- Support staff development opportunities and engage staff appropriately in decision making
- Conduct probationary and annual performance reviews
- Support the development and maintenance of a strong team
- Work with program staff to resolve problems as they arise
- Coordinate team meetings
- Monitor attendance and address concerns
