Our agency is committed to taking the journey together and supporting those with whom we work to grow, thrive, and contribute to society.

**About the Role**:
This role will work with both the Rural Wellington Addiction Service (RWAS) and the Fergus Rapid Access Addiction Medicine Clinic (RAAM) to provide peer support services to clients/patients. Both programs are provided through multi-agency collaborations. This community based service provides substance use supports to residents of Wellington County.

The RAAM is a specialized medical clinic that works with patients of all ages to address health concerns related to substance use. The Peer Outreach Worker will play a vital role as a member of our multi-disciplinary teams by offering a range of practical and emotional supports from the perspective of their lived experience, and embodying a sense of hope and possibility for the future

**Role Type**:Full Time (Contract - approx. 6 months)

**Location**: Guelph and Rural Wellington area

**Hours**: Days, Monday - Friday

**Qualifications**:

- Lived experience addiction and recovery required (mental health recovery an asset)
- Completion of post-secondary education in addictions, social services or related field required
- Minimum of two years’ experience providing support to individuals experiencing addiction / concurrent disorders
- Front line experience providing crisis management, outreach services, and community-based support
- Valid G Driver’s License (G2 level or greater); Reliable vehicle and willing to transport clients
- Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) have declared scope of vehicle use to Insurance provider
- Standard First Aid/CPR Level C
- CSC Clearance conducted by PWGS Canada
- Flexible schedule as required to support program delivery; Some evening and weekend work may be required
- Travel throughout Guelph and Rural Wellington will be required

**Role Overview**:
Peer Support Services in RWAS and RAAM:

- Delivering support in a manner that is consistent with emerging best practice and that reflects key principles of the harm reduction and recovery-oriented approaches to care
- Engaging in purposeful and deliberate sharing of your own lived experience of addiction/mental health recovery to facilitate the development of strong, peer support connections with program participants

Deliver peer support services for clients in Wellington county including:
- Supporting program participants in achieving the wellness goals they have identified for themselves in relation to substance use, mental health, and other domains of life (may include maintaining housing, life skills, budgeting, etc.)
- Assisting participants to connect to local resources by providing service navigation support and appointment accompaniment where necessary
- Assisting participants to develop meaningful connections to their community, and facilitating community building with program participants and the Wellington and Guelph communities including neighbors, community groups, resources, etc.
- Working effectively and in collaboration with the broader community mental health and addictions services
- Co-facilitating psycho-educational and support groups
- Crisis planning and intervention as required

Deliver peer support services for patients of the Rapid Access Addiction Medicine Clinic:
- Providing supportive engagement when patients arrive and are waiting at the clinic and/or throughout the consultation process, as required
- Working closely with the multi-disciplinary team to support effective clinic flow
- Contributing to ongoing program and peer role development work- Participating in program development activities as required including program evaluation, data collection and input, process improvement, etc.
