- The Patient Services Supervisor is an innovative leader who supports the daily operations of the _**_St. Joseph’s Home Care’s _**_Hospital to Home Program _**_delivered by St. Mary’s General Hospital in partnership with Bayshore Integrated Care Solutions _**_program while driving a culture of safety and performance excellence, staff engagement and exemplary patient experience. The Patient Services Supervisor will assist and lead in operationalizing this program to its fullest. In consultation with the Manager, the Patient Services Supervisor is responsible for the oversight of program operations. The individual will provide team focused leadership, engage in positive human resource practices and ensure that the program is a leader in clinical standards and practice. _
- The Patient Services Supervisor is accountable for achieving corporate objectives as they support the organization’s mission, vision and values. This leadership role is responsible for the daily operations under their leadership portfolio to ensure effective and efficient services are provided to our patients throughout our community. This role fosters the opportunity to promote positive patient outcomes in the community by developing innovative approaches, strategic vision and leadership expertise. _
- Reporting to the Manager of Patient Services, this position is responsible for the supervision and development of staff and evaluating the ongoing effectiveness of the program. The Patient Services Supervisor provides consultation and advice for process implementation and change initiatives. This role will also assist in leading change initiatives, working towards the standardization and alignment of processes across the organization. The Patient Services Supervisor will collaborate with internal and external partners and, in conjunction with the Director and Manager to support strategic plans and corporate objectives. The incumbent will provide leadership to front-line staff to ensure exemplary experience for the patients and families we serve. _

**_Program Operations _**
- _Ensure that the essential elements of professional practice standards are met in the planning and operationalizing of the program. _
- _Assists in supervision, administrative management, facilitation, and monitoring of the day to day functions of program; including availabilities, staffing and scheduling/ scheduling programs _Advising staff to provide answers to questions and program goals; referring to appropriate person when unable to respond. _
- _Assists in the development, implementation and/ or revision of program goals and objectives. _
- _Embodies the mission, vision and values of the organization and enhance the patient experience, enhance provider/staff experience, improving value and improving _
- populations health to support continuous quality improvement in daily work _
- _Exemplify, embrace and intentionally promote an inclusive work environment where all are meant to feel they belong _
- _Continually demonstrate a commitment to create a positive culture of equity, inclusion, diversity and anti-racism _
- _Participate in the Selection, orientation, education, and provide overall supervision and mentorship of assigned staff, utilizing performance management, coaching, and exercising progressive disciplinary measures when necessary _
- _Ensure staff access and utilization of all available resources to maximize efficiency in the provision of services. Participates and makes contributions as a member of various committees in order to provide solutions for system improvements. _
- _Act as a resource to staff and assist in problem solving, ethical dilemmas and dealing with complaints/concerns to facilitate the best possible resolution _
- _Assist staff in change initiatives such as automation, reform measure implementations, and service provider transitions, etc. _
- _Work with colleagues to review and assign appropriate workload and adjust staffing to ensure efficient program operations _
- _Identify key elements for the success of staff in their job responsibilities and provide education and support in the development of identified core competencies _
