**About the Opportunity**

The **Pay Case Manager** implements and coordinates program activities and liaises with community organizations to ensure consistency, safe and efficient group activities for youth, mentors, students, and community organizations. The Case Manager also supports youth in achieving their goals and that the program achieves its objectives.

Duties and responsibilities are carried out within a trauma and resiliency-informed framework that promotes The Links’ Therapeutic Model of Care, including the organization’s Mission, Guiding Principles, and the Circle of Courage values. Duties are performed within a team environment that focuses on establishing collaborative and trusting relationships with one another and with the organization’s clients. Providing culturally competent services and ensuring client safety, choice and control are priorities at The Link.

**PRIMARY DUTIES AND RESPONSIBILITIES**:
Duties and responsibilities of the **Pay Case Manager** include but are not limited to:
**Coordination**:

- Receives referral and background information from approved referral sources;
- Conducts intake and assessment of participants with a participant-oriented, strengths-based perspective;
- Coordinates communicates, and monitors case plan to encourage successful completion of the program
- Closes files and forwards to appropriate source;
- Acts as an advocate on behalf of the best interest of youth, ensuring their rights are respected;
- Oversees scheduling of youth in the program;
- Documents all contacts and information as per guidelines;
- Completes monthly reports;
- Liaises with community agencies to develop partnerships based on program and participant needs;
- Implements and adheres to all agency and program policies and procedures;
- Identifies and communicates concerns and updates regarding community programming;
- Attends and participates in program meetings.
- Other duties as assigned.

**Participants**:

- Makes links and connections with participants in the community to support their skill development utilizing volunteer work and participant goals;
- Follows youth’s case plan in the community and makes links between the case plan and community activities with the participant;
- Acts as an advocate on behalf of the best interest of the youth and acts to ensure that their rights are respected;
- Acts as a positive role model for youth;
- Ensures all participants receive support, encouragement, and follow-up.
- Manages caseload, including intake, scheduling, and follow up

**Volunteer Mentors**:

- Ensures all volunteers, students, and new staff receive support, training, and direction in regard to the youth;
- Supervises mentors while working in the community with youth;
- Provides feedback and evaluation to mentors, summer team staff, and students.

**Community**:

- Addresses work center concerns with appropriate personnel when necessary and in a timely manner;
- Actively develops positive agency relations with work centers and community groups.

**QUALIFICATIONS**:

- Post-secondary degree in Education or Human Services field;
- Minimum of two years of related experience working with high-risk youth;
- Demonstrated experience in group facilitation.
- Emergency First Aid and CPR Level C;
- Satisfactory Criminal Record Check with Vulnerable Sector Search, a clear Child Abuse Registry check with acceptable Prior Contact checks, and a clear Adult Abuse Registry check, all done within the last 3 months;
- ASIST Certificate (Applied Suicide Intervention Skills Training) and NVCI Certificate (Non-Violent Crisis Intervention Training) preferred or willing to obtain;
- Willingness to obtain necessary training as per agency training policy;
- Knowledge of Indigenous languages and cultures would be considered an asset;
- Knowledge of Strengths-based and Solution Oriented approaches and community resources;
- Crisis intervention, harm reduction, and conflict resolution skills;
- Strong organization, time management, problem-solving skills, and excellent communication and documentation skills.
