The Case Manager is responsible for the planning and implementation of individualized case plans for youth accessing Tina’s Safe Haven, aimed at increasing participant knowledge of and capacity to transition to independence. The Case Manager assumes all duties and responsibilities in accordance with the organization’s philosophy, direction, and policy standards.

**General Responsibility**:

- Engage youth and their workers in development of case plans that identify the needs, set goals, and determines priority actions in developing a strategy to support the youth in meeting these needs and achieve desired goals.
- Monitor progress of youth through regular, structured meetings.
- Provide resource information, complete referrals, and act as an advocate for youth in accessing supports and resources.
- Develop and maintain strategic partnerships with youth serving organizations, resources, and services to build a network of support for youth accessing Tina’s Safe haven.
- Collaborate with Youth Support Workers and other supports to develop skills and knowledge to support independence, work towards personal goals, and respond to any challenges that may arise.
- Develop and maintain positive relationships with youth based on trust, respect, and confidentiality.
- Respond to crisis/emergent situations as required; review incident reports.
- Participate in Ndinawe's on-call rotation
- Ensure appropriate documentation as required through ongoing review of daily logbook, incident reports, and updating youth case files.
- Maintain program statistics, complete written reports, and complete evaluation processes as required; submit written reports to Manager and/or Programs Director as required.
- Work collaboratively with all Ndinawe supports to ensure the needs of youth are being met.
- Attend staff meetings and participate in staff development as directed by the Manager.
- Maintain workspace and participate in general cleanup of the immediate workspace.
- Understand and be committed to culturally centered service delivery reflective of Indigenous values.
- Ensure youth coming to Ndinawe are welcomed and provided an introduction and orientation to all the organization’s programs and services.
- Advocate on behalf of youth with other external systems.
- Maintains healthy and appropriate boundaries with youth either past or present.
- Follow all policies and procedures as outlined in the Personnel Policy and site-specific manual.
- Performs other tasks consistent with this job description as per personnel policies and procedures that aligns with the support and growth of Ndinawemaaganag Endaawaad Inc.

**Qualifications Required**:

- A combination of appropriate education and employment experience.
- Demonstrated case management experience.
- Knowledge of and sensitivity to issues related to youth at risk for or experiencing exploitation, homelessness, or other risks.
- Ability to appropriately assess and respond to situations based on available information and experience.
- Knowledge of community resources and services available to youth.
- Excellent interpersonal and communication skills with proactive approach to problem solving.
- Strong organizational and time management skills to work effectively in an environment that requires the ability to meet deadlines and targets.
- Flexible to be able to respond effectively and positively in diverse situations.
- Computer skills essential. Experience with Microsoft 365 apps including Excel, Word, One Drive, Teams, Outlook.
- Ability to establish and maintain effective working relationships with youth, co-workers, and community agencies.
- Able to maintain high level of independence and self-initiative.
- Demonstrated ability to establish relationships with young people.
- A strong knowledge of the developmental needs of youth and the issues relevant to them.
- Ability to work under stressful conditions with an ability to respond effectively and positively in diverse situations.
- Current Criminal Record Check, Child Abuse Registry Check, and Prior Contact Check required.
- Proficient computer skills: MS Word, Excel, Internet, E-mail.
- Valid Class 5 Drivers License.
