The Glebe Centre is a leading Long-Term Care and Senior Outreach home in Ottawa that serves 254 residents and over 1100 people in the vibrant Ottawa community. **POSITION PURPOSE**:
Reporting to the Director of Resident Operations and Compliance the Social Worker is responsible for the planning, developing, implementing, and evaluating a resident focused Social Services Program in accordance with MOLTC standards and other applicable legislations. The Social Worker will support all aspects of admissions within the long-term care home and will perform standardized intake assessments to determine resident needs and the appropriate level of care. Social Workers will identify risks and determine priorities of placement needs, and coordinate admissions including primary case management up to the admission date with the primary focus on the emotional well-being of residents and their families.

**QUALIFICATIONS**:

- Bachelor of Social Work Degree from a recognized university.
- Registered with the Ontario College of Social Workers and Social Service Workers.
- Specialized courses to support areas of clinical expertise.
- Well-developed English language skills, comprehension, oral and written.

**REQUIRED KNOWLEDGE, SKILLS AND ABILITIES**:

- Sound knowledge of social services within long-term care population, MOLTC legislation and independent accreditation standards.
- Knowledge and practice of social work principles and demonstrated ability to provide resident support, both individually and in groups.
- Strong knowledge of community resources and services.
- Demonstrated problem solving ability and communication skills.
- Well-developed computer skills.
- Experience working with individuals with cognitive impairment and/or disabilities.
- Knowledge of HCCSS processes as they pertain to LTC admission.
- Knowledge of Point Click Care or other online documentation systems.
- Demonstrates a commitment to courteous, sincere, and sensitive resident service. Presents a positive and caring attitude in all interactions. Show patience, tolerance, embrace diversity, and presents a positive image of themselves in all professional interactions.
- Demonstrates good interpersonal skills with all whom they interact. All communications, verbal and written, should demonstrate commitment to internal and external customer service and excellence. Effectively sends, receives, and responds to requests while maintaining a high level of confidentiality. A high degree of initiation and ability to work well with minimum supervision.
- Ability to work in a culturally diverse and inclusive environment.

**FUNCTIONS**:

- Oversee all aspects of admissions to LTCH and internal transfers within the LTCH.
- Acts as primary liaison with Home and Community Care Support Services.
- Supports the planning, developing, and organizing of social services programs.
- Conducts capacity evaluations for long-term care placement as required.
- Offers supportive counseling to residents and their families as needed.
- Acts as advocate for residents, assisting with follow up regarding complaints and concerns.
- Assists resident and families with accessing services, resources and/or programs as needed.
- Assists with crisis intervention and conflict resolution as required.
- Advocates and liaises with community and government agencies on behalf of residents and families as needed.
- Assists to identify and access financial and other resources as needed for individual residents.
- Attends post-admission care conferences on a regular basis and attends annual care as needed. Assists to arrange family meetings as needed.
- Participates in resident focused care planning in collaboration with nursing staff.
- Oversees the development of policies and procedures for social services department.
- Provides general tours of the LTCH and responds to enquiries from the community regarding The Glebe Centre.
- Engages in continuous quality improvement activities and provides statistical reports to the Director of Operations.
- Supports the Residents’ Council and Family Council as needed. Acts as Staff Assistant to the Residents.
- Acts as primary supervisor of social work placement students and acts in supportive supervisory role for social service work placement students.
- Is a member of the Interdisciplinary Care and Services Committee (IDC&S) and Behaviour Support Committee.
