**What would my role be?**

Reporting to the Manager, Admissions and Patient Flow, you will:

- Facilitate pre-admission, triage, and coordinate admissions of patients to CCC, Rehab, and Palliative Care. While responsibilities are divided between two individuals (e.g., rehab/palliative and CCC), you must collaborate closely for an integrated approach and cover for each other as needed.
- Maintain effective and efficient processes to facilitate the timely triage and admission of eligible patients, working closely with internal and external partners.

**How do I qualify?**

Required:

- An Undergraduate degree in Nursing or Allied Health from an accredited university.
- Minimum five (5) years of experience in a hospital setting, specifically in complex continuing care (CCC), rehabilitation (Rehab), and/or palliative care.
- Valid professional certification in a healthcare discipline, which could include credentials as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or other allied health professions.
