The Patient Safety Officer is responsible for working collaboratively with senior leadership, clinical leaders, and front-line staff in the PRC for the purpose of measurably improving patient safety and reducing patient harm/errors. This requires the systematic implementation of effective, reliable care practices and developing and fostering an appropriate institutional patient safety culture. An important aspect of the Patient Safety Officer position is the ability work collegially and collaboratively with internal and external stakeholders including with both individuals and teams.

Overall:
- Provide leadership and direction to the patient safety strategy including, but not limited to, culture reporting and chart reviews ensuring, amongst other things, document completion for ExD organ offers.- Continuously evaluate and improve patient safety based on internal reviews and external requirements, legislation, and standards.- Collaboratively develops a comprehensive Transplant Services Patient Safety Plan in conjunction with internal and external stakeholders.- Monitor, evaluate, and update the Transplant Services Patient Safety Plan as needed.- Conduct an annual assessment of patient risks and collaborate with internal and external teams to update/develop patient safety goals.- Develop and implement a Transplant Services patient safety measurement strategy.- Enhance surveillance systems/processes to identify and mitigate potential patient harm.- Work collaboratively with clinical leaders, physicians, quality team members, and senior leaders to develop patient safety education for front-line staff.- Collaborate with the Quality Department to develop safety huddles, coordinate root cause analysis of patient safety events, and ensure implementation of action plans.- Provide input into budget planning related to patient safety.- Perform other duties as assigned.

Coordination/Collaboration:
- Builds relationships with the transplant community and partners to remain current on best practices, issues, and trends.- Collaborates with experts.- Collaborates with internal stakeholders to support operational activities around organ and tissue allocation.- Collaborates with OH colleagues to support and execute other activities as assigned.

Relationship Management:
- Develops relationships with colleagues within the team, across the organization, and externally to understand best practices and issues, and to share information.- Demonstrates leadership and integrity in interactions with internal and external stakeholders.- Collaborates with internal colleagues to ensure quality standards and other quality products are delivered on-time and on budget.

Here is what you will need to be successful:
Education and Experience- A Bachelor’s degree is required.- Master’s degree preferred.- Minimum of five years of clinical experience required.- Prior leadership experience with demonstrated success in implementing change within a complex environment required.- Knowledge of rapid cycle improvement theory and human factors principles required.- Patient safety training/education preferred.- Experience in the Provincial Resource Centre is an asset.
