Human relation officer - Long term placement

STATUS: Permanent full-time position (35h / week)
IMMEDATE SUPERVISOR: Head of programs of physical rehabilitation and home care
STARTING DATE: As soon as possible

DESCRIPTION:
Under the authority of the manager of physical rehabilitation and home care, the person ensures a role of liaison between the north and the south for the users in long-term facilities placement for the clientele ID-ASD-PD and loss of autonomy. This person contributes to the organization and planification of the visits. Also, they will work to provide a continuum of care to ensure cultural safety and maintain family ties.

His or her principal intervention areas are as follows:
Clients down South
Assess the needs of the users regarding cultural safety and maintaining family ties;
Participate in the PII for the users;
Work in collaboration with the interveners from the resources and the interveners up NORTH;
Ensure the liaison between the resources north and south according to the different needs of the clientele;
Assiste au réunions interdisciplinaires, discussions de cas et comités divers;
Participate in multidisciplinary meetings, case discussions and other committees;
Any other task related to the position.

Family visit down south:
Coordinate the family visits to Montreal;
Work closely with the team up north to organize and support the family while visiting.

Cultural safety aspect:
Ensure the link between local community resources and northern resources;
Facilitate participation in the traditional activities;
Offer information regarding culturally adapted practice.

**Qualifications**
Must hold a Bachelor's degree in Social Work, Psychology, Psychoeducation, Sexology or Criminology;
Member of his Professional Association;
Minimum of two years of experience working with ID-ASD-PD and loss of autonomy clientele;
Knowledge of Inuit culture and values;
French and English bilingualism required both oral and written, knowledge of Inuktitut will be an asset;
Ability to work in concert with the various stakeholders in the communities;
Ability to work in an interdisciplinary team;
Available to travel to different long-term facilities;
Must have a valid driver's license.

SALARY: Min : 809.20 $ Max : 1 176.35 $ /week
