Under the authority of the Medical and Physical Health Coordinator, you, in collaboration with partners in the region, will perform the necessary functions to coordinate, establish and maintain quality medical and physical health services in terms of accessibility, continuity and safety, in accordance with regional and national guidelines.

You will undertake the activities necessary to identify and assess regional resources and needs. You will design and propose organizational plans to ensure adequate service planning. You will provide advice on implementing or improving services.

You will coordinate the implementation of physical health service corridors between the various levels (local, regional and academic), the development of service agreements and report on the continuity of services provided to the public.

You will also assume specific mandates and act as a contact person with the teams of institutions in the region and the Health Board, always with a view to improving continuity in the service offering.

You will also be responsible for:

- Regional responsibility for Housing, Home Support and Rehabilitation;
- Lead and operate various committees and working groups;
- Attend HSS meetings for the various files under his responsibility;
- Ensure necessary communications, including the preparation of documentation (agenda, minutes, document, action plan, decision-making formula) in order to inform the various authorities in order to better coordinate their actions;
- Collaborate in the development of tools needed to update guidance, prepare achievements and participate in systematic analysis of results;
- Carry out analyses and follow-up of specific files at the request of the MSSS and the other directorates of the Health Board;
- Collaborate with team members and other directorates;
- Perform any other tasks related to the request of their immediate manager.

**Qualifications**
Bachelor’s degree in an appropriate discipline (humanities or health sciences);
**Experience required**
Experience in the health and social services network specifically in the area of housing, home support and rehabilitation

**Conditions and benefits**

One-year temporary full-time position with the possibility of extension (35h/week)

Telework position with regular travel to Nunavik

Salary according to the norms in effect in the health and social services network (27.08$ to 47.98$/h)
- Northern benefits shall be provided pro rata to the time spent in the North
