POSTION: HOSPITAL TO HOME COORDINATOR
HOURS**:MONDAY - FRIDAY 8:00 A.M., - 4:00 P.M.
SALARY: $100,523.00 (MIN) - $125,717.00 (MAX)
As a **Hospital to Home Coordinator**, you will support the development, implementation, and
monitoring of an integrated hospital to home program in collaboration with NWOHT, service
providers and other relevant partners. The position offers an exciting opportunity to play a key
role in reshaping healthcare services and enhancing the well-being of individuals and families in
the Nipissing district.
**ESSENTIAL DUTIES **
Work closely with the interdisciplinary team in reviewing referrals, sending referrals,
attending rounds and ensuring barriers are reduced for patients to move through the
program.
Assess and prioritize each potential referral - directing to the lead agency for those that
qualify while redirecting to other resources such as Ontario Health at Home for those
who do not.
For Hospital to Home patients, and in collaboration with patients and their families,
assess care needs, determine eligibility for services, and develop individual care and
service plans in collaboration with the Service Provider Organization (SPO)
Link patients with service provider
Coordinate and monitor care plan delivery
Establish a helping relationship with patients and their families
Balance patient needs and choices with available resources, ensuring patients’ values
and preferences are respected
Monitor and report on key patient indicators in collaboration with SPO
**QUALIFICATIONS**:
- Bachelor’s or Master’s degree in a health related field required
- Regulated Health Professional, in good sanding required
- 2+ years of recent experience in health or a related field required
- Knowledge of the health care delivery system and community resources
- Demonstrated leadership, change management, problem solving, critical thinking,
conflict resolution, team building and communication skills
- Effective time management, prioritization and organizational skills, with the ability to
work independently and co-operatively in a busy multidisciplinary environment
- Established ability to accurately complete required documentation, reports and forms
- Proficient in a Microsoft Word, PowerPoint, and Excel required and HIS expanse
- Good attendance and work record required.
