The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

**QUALIFICATIONS**
- Current registration with the College of Nurses of Ontario
- Successful completion of a BScN in Nursing; Masters degree preferred
- Current Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS ) certification
- Critical Care experience minimum 10 years required
- Strong interpersonal and communication skills
- Excellent organization and time management skills
- Exceptional decision making and problem solving skills
- Demonstrated effective problem solving and critical thinking skills
- Demonstrated and proven ability to work in a collaborative care model with all members of the health care team including patients and families
- Demonstrated commitment to patient safety
- Commitment to continuous improvement and learning.
- Demonstrated resilience to adapt in an ever-changing environment.
- Excellent attendance record.
- High degree of accuracy and attention to detail.
- Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis.
