The Garry Hurvitz Centre for Community Mental Health at SickKids (CCMH at SickKids) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids). CCMH at SickKids is undertaking an ambitious agenda to establish a revitalized academic community mental health centre that provides the highest quality of care, combined with excellence in teaching and community-based research. CCMH at SickKids is located across two sites in Toronto and offers both in-person and virtual care to clients and their families, from prevention and early intervention through assessment, counselling and therapy and intensive services. We are also connected to the continuum of mental health services for children and youth supported by SickKids: the Brain and Mental Health Program, the SickKids TeleLink Mental Health Program, the SickKids CCMH Learning Institute and our online resources through AboutKidsHealth. **KEY RESPONSIBILITIES**

**Community and Home Services**
- Provides community-based primary prevention and early intervention services to children (ages 0 - 6) and families in community setting
- Enhances parenting skills building on parents’ strengths, referring to appropriate programs, connecting them with resources in the community
- Collaborates with community members providing guidance, planning and expertise in order to assist community members to affect positive change
- Provides a safe environment that protects children from physical and emotional harm
- Work in collaboration with a Public Health Nurse and other professionals, as required
- Plans and prepares materials for child development education demonstrating childcare techniques in group settings and preparing relevant documentation
- Provides instruction and assistance, responds to questions, models effective behaviours and discusses community issues with parents
- Reviews group with the Program Supervisor, as needed
- Maintains up-to-date knowledge of trends, and best practices within the industry
- Makes decisions related to community-based primary prevention and early intervention services within established policies and guidelines

**Other**
- Complies with the organization’s vision, policies and procedures
- Performs other tasks as assigned by management

**Effort**
- Concentration and creativity required daily to plan listen, provide play and learning activities, prepare documentation, etc.
- Sits, stands and walks to perform duties of the job, and lifts up to 10 pounds (groceries) and sets up rooms for meetings (tables, chairs, etc.)
- Travel by car or public transportation to various home/community sites (10-15km)

**Qualifications**
- High School graduation and a minimum of one years’ related experience or equivalent combination of education and experience
- Ability to read, write and speak fluently in another language.
- Good verbal and written communication skills to effectively deal with sensitive and confidential child and family issues
- Good problem-solving skills to identify and report serious issues
- Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
- Certification in First Aid, CPR.
- Knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as office (telephone, scanner, photocopier and fax).
- Ability to work flexible hours
- Ability to travel to conduct home visits 10-20 km)

**Working Conditions**
- Works in a community sites and homes (80%), as well as pleasant office environment (20%)
- Exposure to lack of privacy and monotony
- Little exposure to hazards
