At Stratford Perth Shelterlink, our mission is to assist homeless youth and those at risk of becoming homeless. We do more than just assist in finding them a warm safe bed — we are actively working with youth to assist them in their pursuits, through education and skills development supports.
Duties & Responsibilities
- Reporting to the Executive Director, the Living Options for Youth (LOFY) Case Manager will be responsible to develop and oversee the case management plans for youth accessing Shelterlink;
- Collaborate with Executive Director in program delivery through an Anti-Racist, Anti-Oppressive, Client-centered, Housing-First lens;
- Administer assessments to youth (e.g., VI-SPDAT) and initiate appropriate next steps including diversion, family reconnection, shelter intake, etc.;
- The Case Manager will conduct intakes/discharges and provide ongoing support to youth in LOFY based on an individual Plan of Action with established short-term and long-term goals;
- Ensure smooth, effective, and efficient operations of the LOFY Program including responding to client issues and client crisis;
- Participate in staff meetings, youth meetings, and program requirements to operate consistently and effectively;
- Keep open communication lines with the Executive Director, Program Coordinator, and Outreach Case Manager to ensure awareness of concerns, changes, activities, etc. including staff performance;
- Assist LOFY participants with Ontario Works, Ontario Disability Support Program inquiries, Choices for Change, public health, and other referral sources based on individual needs;
- Complete other monthly documentation and statistics tracking/collection as requested within established deadlines;
- Provide guidance, advocacy, and life coaching through dialogue and through role modeling of values and ethics relevant to the provision of services for at-risk youth;
- Assist in orientation of new employees and provide supervision of assigned students on placement at Shelterlink;
- Respond appropriately to difficult behavioral situations, demonstrating ‘good judgment’ in emergency situations;
- To adhere to established Shelterlink goals, philosophy, protocol, policies, and procedures;
- Collaborate with staff on duty to provide supervision of LOFY residence at all times;
- Coordinate with Executive Director & LOFY staff to provide feedback when meeting on related issues or topics;
- Attend and represent the agency in community meetings and raise community awareness by assisting in the organization and implementation of Shelterlink fundraisers;
- Coordinate, liaise, and collaborate with day-to-day operations, LOFY office activities, SHOPP program and After-Hours staff and administration;
- Provide additional public support to Executive Director via public speaking engagements.
Qualifications & Requirements
- Degree in Social Work (preferred) or Diploma in Child and Youth Services or Social Services, Psychology or Education;
- Equivalent work-related experience, administration and supervisory experience is an asset;
- Minimum of one (1) year of previous or similar experience with legislation, regulations, policy & procedures for program development & implementation AND involvement with youth in community programs;
- Sound organizational and time management skills;
- Excellent attention to detail
- Demonstrated advocacy skills and recording in confidential situations
- Basic team building, negotiating & mediation skills
- Thorough knowledge and experience working with vulnerable and marginalized populations from a harm reduction perspective and a trauma informed lens;
- Knowledge of Perth County resources;
- Knowledge or experience with programs and/or services as related to the housing first for youth philosophy;
- Proven ability to communicate effectively both orally and in writing
- Enhanced computer literacy and office administration including experience in Microsoft Word, Outlook, and Excel.
