Calgary, Canada Cerebral Palsy Association in Alberta Full time

**Objective**: To advocate for persons with disabilities and assist to foster their capacities and coping mechanisms to improve daily lives and experiences.

**Summary**: 1 Year Temporary Contract with possibility to extend. - Responsible for intake coordination, assessment of new members, building a supportive relationship with each client/member and utilize a client centered approach to guide individuals with disabilities and those who support them to live life without limits.

**Responsibilities**:
**Programs and Support Services**
- First point of contact for clients wishing to join the CPAA as a client/member
- Initiate intake process, ensure all information has been collected and recorded, send welcome package.
- Meet with clients for initial needs-assessment and determine appropriate programs, services, referrals.
- Conduct/update environmental scans for housing, employment, funding etc. for people with disabilities.
- Update client case notes in the appropriate data base platform
- Provide referrals to appropriate resources, programs, or services, internally and externally
- Provide general information/education about disabilities
- Coordinate CP Cares program
- Coordinate social, educational, and life-skills workshops for clients, parents and families
- Coordinate support services programs (e.g. early intervention therapy program)
- Represent CPAA at community events and resource fairs
- Provide supports in creating and providing community presentations to educate and bring awareness about disabilities and CPAA
- Coordinate community outreach in order to create relationships that will support Programs and Services offered to clients and their families
- Support Director of Social Inclusion and Support Services in building community relationships around the province

**Administrative**
- Support Director of Social Inclusion and Support Services with creating reports and collecting data
- Keep clients case notes and community outreach notes up to date

**Other**:

- Carry out tasks in accordance with health and safety standards and policies
- Complies with CPAA policies and procedure
- Perform other tasks as assigned by management

**Working Conditions**:

- Interacts with students, family members, staff, visitors and government agencies.
- This is not a remote position, working from the office is required
- Manual dexterity required to use desktop computer and peripherals
- Flexibility required to standard working week (Evenings/Weekends on occasions)
- Intermittent physical activity including walking, standing, sitting and lifting.

**Qualifications**:

- Social Worker or Counsellor diploma required, degree would be an asset
- Experience with intake, and service coordination in the disability community
- Knowledge of community resources and other disability agencies as well as systems navigation (PDD/AISH/FSCD) will be an asset
- Ability to work individually and as part of a team
- Experience with facilitating group sessions/workshops/program coordination
- Knowledge of and proficiency in use of word processing, database software, record keeping
- Ability to organize & prioritize work, and to manage multiple projects at a time
- Advanced written and communications skills
- Knowledge of and proficiency in use of online communication and meeting platforms, including ZOOM, Teams, Google Meets
- Driver’s license and vehicle - travel within the province may be required

Pay: $43,000.00-$47,000.00 per year
