- Est capable d’établir une relation avec une clientèle francophone et anglophone;
- Possède un permis de conduire valide et un véhicule pour se déplacer;
- A une connaissance de la politique et des programmes en SAD ainsi qu’auprès de la clientèle gériatrique;
- A une expérience professionnelle dans la gestion de projets, un atout.

- Le CIUSSS de l’Ouest-de-l’Île-de-Montréal (ODIM) recherche actuellement un(e) agent(e) de planification, de programmation et de recherche (APPR)
- L’APPR exerce les fonctions et l’autorité qui lui sont déléguées par l’adjointe à la directrice, DSAPA, volet soutien à domicile (SAD). Votre contribution pourrait être sollicitée dans une variété stimulante de projets selon les grands thèmes du soutien à domicile et des services de proximité, de la fluidité de la trajectoire, de la vigie des milieux de vie privés et des services spécialisés en gériatrie. Votre contribution sera précieuse en ce qui a trait à l’amélioration des processus cliniques et dans la planification des services offerts aux usagers du territoire et ce, dans les différents secteurs.
- Horaire : lundi au vendredi, de 8h à 16h
- Vos principales responsabilités:
- Participer à l’élaboration des plans d’action dans le cadre des mandats qui vous sont confiés;
- Assurer le suivi administratif (rédiger des états de situation, préparer des présentations, rédiger des descriptions, proposer des plans d’action,...) de l’ensemble des étapes d’opérationnalisation des programmes dédiés aux services de soutien à domicile;
- Participer à l’élaboration des outils de suivi de gestion nécessaire à la reddition de comptes;
- Préparer les contenus des rencontres (avec les organismes communautaires, les regroupements régionaux, les partenaires du milieu, etc.) et assurer les suivis;
- Proposer des critères, des indicateurs ou une méthodologie permettant l’évaluation d’atteinte des objectifs ou l’allocation des ressources financières;
- Participer aux travaux de groupes de travail ou sous-comités en fournissant les données requises et en présentant des solutions aux problèmes identifiés;
- Réaliser des évaluations de processus ou de programme et émettre des recommandations à la lumière des résultats obtenus;
- Participer au développement des mécanismes de communication et de consultation auprès des organismes communautaires, des regroupements régionaux, des CIUSSS et autres partenaires du milieu;
- Soutenir l’assurance qualité avec les principaux fournisseurs des services à domicile ou des milieux de vie de type ressources intermédiaires ou résidence pour personne âgée.
- Soutenir l’innovation: expérimentation de formules neuves dans le but de mieux répondre aux besoins.
