Job Title: _Case Manager Clinician_

Summary:
The Victoria Youth Clinic Society, lead agency and operator for Foundry Victoria is seeking a Case Manager Clinician, to support our allied health and Family reconnect program.

In accordance with a client and family centered model of care and, in accordance with the Mission, Vision and Values and strategic directions of the Victoria Youth Clinic Society and Foundry, this position promotes a safe, respectful, and civil working environment for clients, families, visitors and staff. Foundry is removing barriers and increasing access to health and wellness services for young people ages 12 - 24 and their caregivers through integrated mental health care, substance use and harm reduction services, physical and sexual health care, youth and family peer support and social services.

Reporting to the Team Lead and under the direction of the Clinical Coordinator, the Case Management

Clinician works with youth and families experiencing homelessness, substance use and mental health challenges, and those with difficulties accessing health care services. The clinician works independently, and as part of a robust interdisciplinary team of primary care, substance use and mental clinicians and peer support workers to provide advance level clinical interventions. The Case Management Clinician conducts assessments, coordinates care, supports patients to reach their goals through relationship based trauma informed interventions such as outreach support, harm reduction strategies, treatment options, therapeutic interventions, and community referrals as per individual client needs.

**Qualifications**:
Post-secondary education in a health-related discipline from an accredited institution is required for this role. Preference given to those with a BSW but other disciplines considered based on equivalent work experience.
- A minimum two years’ experience in fast paced interdisciplinary teams working with youth and young adults;
- A minimum two years’ experience working with youth living with substance use and mental health concerns;
- Experience working in primary care settings considered an asset;
- Strong knowledge of harm reduction philosophy and practice

Skills, and Abilities:

- Demonstrated ability to provide evidence-informed care to youth and families experiencing substance use and mental health concerns, homelessness and who are adversely affected by social determinants of health.
- Demonstrated ability to provide care from a trauma-informed, client-centered, harm reduction and Culturally safe practice;
- Ability to manage crisis interventions, and to actively engage clients with severe substance use concerns who do not readily engage with traditional health care services;
- Solid understanding of substance use and mental health assessments, treatments and interventions, and case management;
- Experience working with Indigenous, BIPOC, First Nation and LGBTQ2+ communities;
- Demonstrated ability to complete screening and comprehensive assessments for mental health, substance use, suicide, high risk behaviours, and psychosocial assessments using standardized frameworks and tools
- Ability to work effectively in team-based settings and independently;
- Effective communication skills, both verbally and in writing;
- Excellent skills in conflict resolution that promotes collaboration and effective problem solving;
- Ability to manage multiple competing demands and organize and prioritize workload;
- Demonstrated knowledge of relevant Acts, including Child and Family Community Services Act,

Mental Health Act and Health Care Consent and Care facilities Act etc.);
- Solid understanding of human services ethics with regard to confidentiality and appropriate boundaries;
- Ability to take initiative and exercise clinical judgment.

Key

**Responsibilities**:

- Engage with youth and their families experiencing severe substance use and mental health challenges through assertive outreach, engagement and effective communication;
- Provide case management and care coordination through screening of referrals, assessment, harm reduction, treatment and monitoring for individual clients;
- Conduct comprehensive psychosocial and substance use assessments to ensure timely and appropriate services to youth and their family;
- Create comprehensive care plans and interventions for youth and their families, using evidence
- based practice interventions and including the voices of youth and their families, where appropriate;
- Utilize and provide a variety of treatment modalities for individuals, groups and families;
- Ensure clients have access to harm reduction supply and distribution;
- Coordinates patient care, including counseling, nursing and primary care services;
- Provides outreach to youth throughout the community, including housing, treatment centres, shelters and community spaces;
- Conducts referrals and supports discharge planning for youth;
