**Position Title**: Registered Social Worker

**Status**:Part-time (37.5 hours bi-weekly)

**Reports To**: Executive Director

**Position Summary**

The Social Worker will function as a member of an interdisciplinary team providing direct interpersonal care to clients by assessing and providing supportive interventions to individuals of all ages, couples, and families. The Social Worker role also includes the planning and facilitation of group counselling and mental wellness sessions.

The Social Worker will actively participate in the development and implementation of health promotion programs with individuals, families, and community groups.

This incumbent will make decisions and accept legal responsibilities, within the scope of responsibility for Registered Social Workers, for all aspects of clinical services required. The Social Worker is expected to uphold and maintain the professional and organizational core competencies necessary for clinical practice in the primary care setting.

**Duties and Responsibilities**

**Generic and/or Specific**:

- **Patient Focused**_
- Assessment and evaluation of the strengths, needs, and resources of individuals and families referred for counselling
- Provision of individual, couples, and group counselling, guided by evidence-based methods of psychotherapy and other therapeutic interventions
- Collaboration with PrimaCare Community Family Health Team (PCCFHT) team members on approach to management of a client’s psychosocial and mental health challenges, when appropriate
- Education for patients and families on the interplay between physical, psychosocial and mental health issues
- Facilitate and provide guidance for clients to access appropriate community resources
- Advocacy when patients experience difficulties accessing services
- Develop and facilitate group workshops for the purpose of skills-development in managing mental health challenges
- Model both professional and the organizational core competencies

- Collaborate when appropriate with physicians and other PCCFHT team members
- Provide consultation and clinical support to peers as needed
- Assist in the orientation and preceptorship of new staff and student learners
- Communicate with community agencies on client care issues
- Ability to work occasional evening hours
- Ability to remain flexible to the unique needs of each Physician/PCCFHT Clinic
- Effectively utilize human and material resources

- Function within the scope of practice as defined by the Ontario College of Social Workers and Social Service Workers
- Adhere to PCCFHT policies and procedures
- Participate in quality improvement and program evaluation projects
- Act in accordance with Ontario’s Personal Health Information Privacy Act to protect clients’ privacy and confidentiality
- Other duties as assigned by Executive Director

**Education, Skills and Experience**

**Education**:

- Master’s degree in Social Work along with one to two years of supervised practical training
- Certification in CBT or MBCT preferred

**Experience**:

- 2 or more years of experience providing care as a Social Worker in a community setting

**Professional Affiliations/Memberships**:

- Current registration with the Ontario College of Social Workers and Social Service Workers

**Additional Skills/Abilities**:

- Ability to work independently for extended periods of time
- Commitment to client-centred care
- Demonstrated ability to work in a collaborative practice model with multidisciplinary teams
- Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills and advanced decision-making, problem recognition and problem-solving skills
- Ability to provide mental health assessment and intervention within a short-term care model
- Experience with program development, quality improvement initiatives, and program evaluation
- Effective organizational, interpersonal and communication skills, and demonstrated negotiation and conflict resolutions skills
- Excellent written and verbal communication skills
- Knowledge of Microsoft Office and Electronic Medical Records
- Recognition of the limitations of own competencies and when to consult or refer to another healthcare provider

- Ability to sit for occasional extended periods of time
- COVID-19 Vaccination required
