**Job Summary**:
The Behavioural Supports Ontario (BSO) Long Term Care (LTC) Lead Assistant will serve as an in-house expert on identification and management of responsive behaviors. This dedicated lead will function in a specialty role providing behavioural support expertise to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.

**Duties & Responsibilities**:

- Work in collaboration with the Lead to identify triggers and supports the development of behaviour care plans and approaches to providing care to residents requiring behavioural support as well as their families.
- With the Lead, mentors coaches and collaborates with long-term care staff “in the moment” to minimize and/or mitigate client behavioural responses.
- Together, with the Lead, facilitates (weekly and as needed), client behavioural rounds in collaboration with clinical team members as required.
- Collaborate with the Divisional BSO team, Geriatric Mental Health Outreach Team, Psychogeriatric Resource consultants and mobile behaviour support resources (i.e. PRCs, NP Stat, System Navigator, Community Support Worker, etc.), primary and acute care to provide holistic, wrap around care.
- Attend all necessary care conference meetings related to residents with responsive behaviours, as required
- Provide support to unit teams in preventing and managing responsive behaviours.
- Aid in the implementation and coordination of the comprehensive transition plan to support the transition of a new or returning residents with responsive behaviours into the home.
- Provide hands on coaching and mentoring of staff in effective approaches to care.
- Identify and flag residents who require additional attention.
- Document all findings and interventions in a clear and concise manner in the client’s clinical record.
- Advocate for and, protect the individual rights of, residents/clients.
- Maintain a current knowledge of Alzheimer’s disease and related dementias, mental health and addictions and neurological conditions.
- Other duties as assigned.

**Skills & Qualifications**:
**Essential**
- PSW certificate from a recognized college.
- Three to five (3-5) years’ experience with residents in a long-term care home setting.
- Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.
- Ability to approach highly sensitive issues with professionalism and tact and a demonstrated ability to coach and mentor others.

**Preferred**
- Training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility.
- Basic computer skills - Microsoft Word, Excel and online learning utilities including ability to use electronic communication devices.
