About the role
As Community Wellness Initiatives Facilitator supports the health and wellness of Aboriginal individuals, families and communities through the promotion of healthy living activities, including the delivery of workshops and healthy living groups. The position plays an integral role in improving Chronic Health Conditions by facilitating support in community development and engagement initiatives, as well as provide input to, coordinate and support community health and wellness initiatives and health literacy.
Areas of focus include healthy eating, food security, physical activity, increasing awareness of chronic health conditions including risk factors, pain and disease management, and improving access to services and screening. The position works collaboratively with all health partners and program specialists within the Fraser Salish Region.
This position is a shared initiative with Fraser Health Authority.

PRINCIPAL DUTIES AND RESPONSIBILITIES
- Promotes health and wellness through the delivery of workshops, healthy living groups and other opportunities as identified with communities and Health Standards Organization (HSO).
- Prepares and delivers presentations and workshops, promoting chronic disease prevention and healthy living.
- Collaborates with Wellness Nurse Navigators and program specialists on a range of health promotion and disease prevention activities
- Plans, organizes, coordinates and participates in the development and evaluation of community health and wellness education and promotion programs. Provides input to the establishment of best practices
- Provides input and recommendation to the development and distribution of educational and informational materials on chronic health issues to First Nations communities and urban Aboriginal organizations
- Maintains cooperative working relationships with partner agencies to coordinate healthy living promotion activities

About you

To be successful in this role you will have/be

Education
- Bachelor's degree in public health, kinesiology, exercise management, health education, nursing, social work or related human services field, or an equivalent combination of education, training and experience
- Training in cultural safety and humility, motivational interviewing and trauma-informed care an asset

Experience
- Significant, recent and relevant experience working in a health field (usually acquired over a 4-6 year period)
- Experience working with Aboriginal individuals, communities or organizations on issues related to health and wellness
- Experience planning, implementing and evaluating programs and services

Knowledge
- Extensive knowledge of public health, chronic diseases, nutrition, and health promotion
- In-depth knowledge of Aboriginal/First Nations health and wellness system needs and determinants of health
- Excellent knowledge and understanding of Public Health systems and Aboriginal health systems in BC
