The
**Spiritual Health Practitioner**
- **Palliative Care Program** role provides you the opportunity to oversee the provision of comprehensive spiritual health services to residents, patients, families and health care staff within Fraser Health facilities in a manner that is appropriate to belief systems and cultural diversity of our populations.

We are looking for a
**Full Time**
**Spiritual Health Practitioner **to provide support to our Spiritual Care and Patient Experience team at our Central City Tower, located in
**Surrey, BC.**

Embracing all faiths, spiritual belief systems and cultural diversity, spiritual health services will focus on identifying and meeting the individual’s spiritual needs and supporting community spiritual/faith based relationships.

Detailed Overview:
**Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families**:
Responsible for providing and/or facilitating spiritual health services by providing support to volunteers, volunteer coordinators and managers and facilitating linkages with a range of local community faith and spiritual groups; utilizes professional and program standards to provide coordinated, comprehensive spiritual services such as liturgical, psycho-social and spiritual support for patients, residents, staff and their families that embraces all faiths/spiritualities/cultures with the focus on supporting the individual's community based spiritual relationships for an assigned location(s); works in partnership and in collaboration with volunteers, volunteer coordinators/managers and appropriate representatives of local faith and spiritual communities in the recruitment, orientation and continuing education of spiritual health volunteers and in the provision of spiritual health at the assigned location(s).

**Responsibilities**:

- Plans, organizes, coordinates and evaluates spiritual care services for residents, patients, families and staff; provides for and/or arranges for spiritual care services to patients, families and staff on a multi-faith, multicultural basis for an assigned location(s); facilitates crisis interventions for staff, patients, resident and/or families, as needed.
- Evaluates spiritual health services and ensures spiritual health delivery is aligned with Fraser Health (FH) policies, procedures and standards and in accordance with Professional Practice and the Provincial Spiritual Health Framework.
- Implements programs involved with spiritual care services for patients, families, friends and staff; provides information regarding spiritual care for families, staff and the community; liaises with appropriate community faith groups to develop and maintain spiritual care services for an assigned location(s).
- Ensures and respects the individual's freedom of choice and privacy to select and/or request/decline spiritual support; provides formal and informal consultations to staff, physicians and members of the interdisciplinary team in verbal conferences/rounds; supplies chart documentation and notation of spiritual care assessment and care plan.
- Ensures information is distributed to patients/residents/staff or families regarding available religious and/or spiritual care services; liaises with representatives of community religious
